Objective:To explore the influence of tripterygium-oral-preparation on osteoporosis through an experimental research of postmenopausal osteoporosis in animal models and clinical application.Methods:Seventy-five3-month-old female SD rats were randomly divided into five groups:ovariectomized group (OVX),three diffetent volume of tripterygium-oral-preparation treatment groups (A, B and C) and sham-operated group (Sham),each group had15rats. The group OVX,A,B and C were ovariectomized while the group Sham closed the fascial and abdominal incision without ovariectomy after the abdominal cavity opened. Before modeling and12weeks after modeling,the BMD of lumbar spine should be detected by dual-energy X-ray and the success model shoule be filtered. A total of five rat models of osteoporosis didn’t meet the standard include each one in group OVX, A, B, and two of group C.Experimental study was conducted in line with the qualified model:the group A, B and C gavaged with tripterygium-oral-preparation with a volume of1.25ml/Kg,2.5ml/Kg and5ml/Kg,once a day with a total treatment of12weeks; the group OVX and Sham gevage with physiological saline with a volum of2.5ml/Kg, once a day with a total treatment of12weeks. After12weeks, detected BMD of lumbar bone, E2, B-ALP, and BGP. Observed tissue morphology of each femoral condyle and reconstructed the L4under Micro-CT.From April2013to October2013, sixty cases of postmenopausal osteoporosis were randomly divided into experimental group and control group inorder to cinducted preliminary clinical observation:the experimental group was treated with tripterygium-oral-preparation20ml/times,1times/day; the control group:oral saline placebo,20ml/time,1time/day. After the trial began, the pain relief scores and liver and kidney function should be reviewed every4weeks; before the test, and when testing12weeks,we detect the patients with B-ALP and BGP, BMD T-score on the same side of the distal radius and ulna;and complications records of patients after treatment.Results:After gavaged for12weeks, the lumbar spine BMD between the groups shows that Sham> B>A>OVX (P<0.05). The E2levels in each group of rats was significantly decreased compared with group Sham, but the A, B and C decreased level were less than group OVX. The levels of B-ALP and BGP of the four ovariectomized group were increased compared with group Sham, but the extent of group A, B and C of B-ALP and BGP increased less than group OVX (P<0.05), and group B has the most significant difference (P<0.01). According to tissue morphology and Micro-CT reconstruction, the trabecular bone of group Sham arranged closely, uniform thickness, without obvious trabecular fracture and bone defect,the continuity of trabecular was good;while most of the trabecular bone of group OVX sparse small beam, it shows clearly visible of trabecular bone fracture and bone defects, the continuity of trabecular bone was poor, the trabecular bone situation in group B were closest to group Sham, while the trabecular bone situation of group A and C is connected between the group B and OVX.Fifty-five patients completed a12-weeks clinical observation. In every4weeks after administration,the pain relief of experimental group was better than the control group (P<0.05). The B-ALP and BGP levels of the experimental group after treatment for12weeks compared with the levels before treatment and the same period of treatment of control groups were significantly decreased (p<0.01); and the T-score of the experimental group after treatment for12weeks compared with the levels before treatment and the same period of treatment of control groups were significantly increased (p<0.01). There was no observed significant liver and kidney damage and other serious complications appeared in the clinical applicationConclusion:The tripterygium-oral-preparation may has the role in anti-osteoporosis.
