Objective:To retrospectively summarize a multi-centre experience and outcome of Mainz Pouch II urinary diversion.Methods:From October2004to October2012, a total of180multi-centre patients (146male and34female, median age57years, range15to74years) who underwent Mainz Pouch II urinary divension procedure have been retrospectively analyzed. Data on early complications were available for all patients, while late follow-up data were available for164patients (91.11%). Operation method, Operative time, Surgical approach, blood loss, Hemorrhage volume, Early complications,Late complications, Urine volume, Continence behaviour, the quality of life, pyelonephritis, and metabolic disorders were investigated.Results:Operations in the group were finished smoothly.6patients died perioperatively. Early complications occurred in48patients (48/180,26.67%), including pneumonia (6/180,3.33%), prolonged ileus (8/180,4.44%), wound infections (28/180,15.56%). The remaining6patients(6/180,3.33%) suffered leakage of the urine/stool mixture from the pouch.3cases healed after adequate drainage, anti-infection and intravenous nutrition therapy, and the other3cases died due to complicated with purulent peritonitis, septic shock, MODS. Recent and late complications occurred in21.95%(36/164) of patients, including2patients (2/164,1.22%) with uretero-intestinal anastomotic stricture;4patients (4/164,2.44%) with hydronephrosis;2patients(2/164,1.22%) with diarrhoea;12patients (12/164,7.32%) required intermittent antibiotic management because of recurrent pyelonephritis.16patients (16/164,9.76%) developed metabolic acidosis and oral sodium bicarbonate supplement required.Conclusion:The Mainz Pouch II is a simple and low risk procedure of continent urinary diversion, with fewer morbidity and complications, excellent controllability and good quality of life. It may be used as a convenient and satisfactory alternative in patients who are not suitable for other types of continent urinary diversion.
