Objective: To examine the serum levels of Interleukin-33(IL-33) and solubleST2(sST2) in patients with heart failure, and further explore the application values andclinical significance of IL-33and sST2in heart failure.Methods: A total of44patients with heart failure admitted in cardiovasculardepartment in Suzhou University affiliated1stHospital from July.2013to Dec.2014wereenrolled. According to the pathogen the patients were divided into3groups, including14,10,20. On the basis of NYHA classification the patients were classified into14,13,17.Meanwhile,24healthy people were enrolled as control, which were selected from Centerof Health Examination. The cardiac ultrasound test (UCG)and plasma NT-proBNP levelsof patients were determined, and all of the clinical data were recorded. Serum IL-33andsST2levels were investigated in both acute stage when patients were admitted to hospitaland stable stage after being admitted to hospital for7-14days in average in heart Failuregroup. Serum IL-33and sST2levels were tested in control group, in which volunteersunderwent health examine that were admitted to hospital.(1) Comparing the serum IL-33and sST2levels in HF group (acute stage, stable stage)and control group; Comparing the serum IL-33and sST2levels in HF group around acutestage and stable stage.(2) Comparing the serum IL-33and sST2levels (acute stage, stable stage) of patientscaused by different factors in HF group.(3) Comparingbetween different causes of heart failure group serum levels of IL-33and sST2(acute stage).(4) Comparing the serum IL-33and sST2levels of patients in different cardiacfunction scales in HF group.(5) Analyzing the relationships in serum IL-33and sST2levels.(6) Analyzing the relationships in serum sST2and NT-proBNP, hs-CRP, NYHAclassification, LVEF, LVEDD and LVEDS.(7) Obtaining the fitting curve of serum sST2and plasma NT-proBNP for diagnosingheart failure.Results:(1)Serum IL-33andsST2expressions of HF group in acute stage were significantly higher than that of control group (p<0.05). Serum IL-33expressions of HFgroup in stable stage were obviously significantly higher than that of control group(p<0.01), Serum sST2expressions of HF group in stable stage were significantly higherthan that of control group (p<0.05). Serum IL-33and sST2expressions of patients in acutestage showed no difference compared to that in stable stage.(2)Serum sST2production inpatients triggered by different causes showed no difference between acute stage group andsable stage group.(3)Between different causes of heart failure in acute stage serum levelsof IL-33and sST2, the difference was not statistically significant.(4) Serum IL-33levelsshowed no significant difference in different cardiac function group.Serum sST2levels ofpatients in cardiac function scale IV were obviously higher than that of patients in cardiacfunction scale II (p<0.01) and III (p<0.017).(5)Spearman correlation analyses revealedthat serum IL-33expressions had no correlation with serum sST2expressions (6)Spearmancorrelation analyses revealed that serum sST2expressions had positive correlation withNT-proBNP (r=0.241, p<0.05)、 hs-CRP(r=0.360, p<0.01) and NYHA classification(r=0.648,p<0.01), and had negative correlation with LVEF (r=-0.356, p<0.01).(7)In ROCanalysis, area under the curve (AUC) of sST2in diagnosing HF was0.694(95%CI:0.571-0.817,p<0.01), and AUC of NT-proBNP in diagnosing HF was0.985(95%CI:0.000-1.000,p<0.01).Conclusion:(1) Serum IL-33and Sst2levels evidently increased in patients with HF,and the sST2levels were obviously related to the severity of heart failure.(2) Serum sST2levels had positive correlation with NT-proBNP、 hs-CRP andnegative correlation with LVEF.(3) Serum sST2would be a new biology marker in diagnosing heart failure, and mightbe an auxiliary method along with cardiac ultrasound and NT-proBNP detection, whichhelps to improve the accuracy of diagnostic and predict the prognosis of HF.(4) The IL-33/ST2L signal pathway have a heart protection, and if we interfer thissignaling pathway,It may be become heart failure treatment of targets.
