Purpose: To explore the association of early changes in apparentdiffusion coefficient (ADC) and venous enhancement (VE)1month aftertransarterial chemoembolization (TACE) with tumor response as assessedby using RECIST6months after therapy.Materials and Methods: A retrospective analysis of48patientswith71hepato-cellular carcinoma (HCC) lesions undergone magneticresonance scanning in2009March to2013July respectively beforeTACE, as well as1and6months after TACE (in the lesion area:1~25mm2). The lesions in6months after TACE treatment was evaluatedand classified according to the Response Evaluation Criteria in SolidTumors(RECIST) or revised RECIST. Analyse the association ofvariation of ADC value and VE value in target lesion before and afterTACE with the therapy effect. Evaluate the classification effect accuracywith receiver operating characteristic (ROC) analysis.Results: According to RECIST criteria,6months after TACE,30HCC lesions showed partial response (PR),35showed stable disease (SD), and six showed progressive disease (PD). Increase in ADC anddecrease in VE1month after TACE were significantly different betweenPR, SD, and PD. At area under the ROC curve (AUC) analysis of theADC increase, there was an AUC of0.78for distinguishing PR from SDand PD (the critical value is51.3%, the sensitivity was70%, specificitywas73.2%) and an AUC of0.89for distinguishing PR and SD from PD(the critical value is29.3%, the sensitivity was83%, specificity was100%). The AUC for decrease in VE was0.73for discrimination of PRfrom SD and PD (the critical value is51.9%, the sensitivity was76.7%,specificity was63.4%) and0.90for discrimination of PR and SD fromPD (the critical value is26.8%, the sensitivity was86.2%, specificity was100%).Conclusion: The extent of the increase in ADC value and decreaseVE value1months after TACE can forecast long term effect of TACE.The multi modal analysis of MR data can be used as a biomarker toevaluate the long term effects of HCC treated with TACE.
