Objective:1.Study on combined therapy in the treatment clinicalefficacy of vulvar white lesions.2.Study on expression, significance andcorrelation of VEGF and CD34in vulvar white lesions before and aftertreated by combined therapy.To discuss the etiology and pathogenesis ofthe vulvar white lesions and to provide theoretical basis for clinicaltreatment.Methods: Collection of62patients with vulvar white lesions, in ourhospital gynecology clinic from January2011to June2012,under theobvious place to take their genital lesions and confirmed for vulvar whitelesions with pathology examination.32cases were vulval squamousepithelial hyperplasia,30cases were vulva Lichen sclerosis.These casesare conducted the expression of VEGF and CD34in the diseased tissuethrough the methods of immunohistochemical.The expression of CD34were counted with micro vessel density (MVD, micro vessel density).62patients were treated with recombinant human epidermal growth factorgel, recombinant human IFNα-2b gel, ointment estriol, according to theseverity,at1,3,6months,1year after the end of the treatment according to the evaluation criteria determined curative effect and the chang, thentake living tissue pathological examination and immunohistochemicalstudy, compare before and after treatment in62cases of vulvar whitelesion of the expression of VEGF and CD34.10cases of normal vulvartissues as control.Results:1.The itching, flexible, color changes after treatment werestatistically significant (P<0.05),after the treatment the genital itching,skin color, elasticity, etc were greatly improved, the total effective ratewas95.16%. The difference the total efficiency of treatment of vulvarlichen sclerosis (LS group) and vulvar squamous epithelial hyperplasia(SH group) was not statistically significant (P>0.05), compared with thecure, squamous cell hyperplasia healing effect healing effect and lichensclerosis were different (P<0.05).2.Before treatment:the expression ofVEGF and the micro vessel density (MVD) in vulvar lichen sclerosis(LSgroup) were lower than normal vulvar skin group, the difference wasstatistically significant (P<0.05), and compared with vulvar squamousepithelial hyperplasia (SH group) the difference was statisticallysignificant (P<0.05);the expression of VEGF and the micro vessel density(MVD) in vulvar squamous epithelial hyperplasia (SH group) VEGFcompared with normal vulvar tissue showed no significant difference (P>0.05); the positive correlation between the expression of VEGF andMVD in the LS was found. After treatment:the expression of VEGF and the micro vessel density (MVD) in LS, SH compared with normal vulvarskin, there were no significant differences between them(P>0.05).Compared VEGF, MVD in LS before and after treatment,thedifferences were statistically significant(P<0.05).Compared VEGF, MVDin SH before and after treatment,the differences were no statisticallysignificant (P<0.05).Conclusions:1.This study confirms that the combined therapy(recombinant humanepidermal growth factor gel, recombinant human IFNα-2b gel, ointmentestriol) has remarkable effect in treating different types of vulvar whitelesions.2.The expression of VEGF and the number of micro vessels in vulvarlichen sclerosis reduced;the expression of VEGF and the number ofmicro vessels in vulvar squamous epithelial hyperplasia was nosignificant change,but there was dysfunction in the structure ofmicrovascular of vulvar squamous epithelial hyperplasia, it wascorrelated with vulvar white lesions.3.After comprehensive treatment, the number and the function of microvessels improved significantly. Tip that local nutritional and metabolicdisorders caused by the disorders of dermis microcirculation wascorrelated with vulvar white lesions.
