Vasopressin Combined Adrenaline On Cardiac Arrest After The Recovery Of Rats Myocardial Damage The Role Of PI3k/Akt Signal Pathway And Its Influence

Objective:When the rats was treated by the vasopressin, epinephrine and their combination after thecardiopulmonary cerebral resuscitation in the asphyxia animal model, the morphologicalchanges of heart muscle was observed though the sections of H&E staining, and theexpressions of p-p38, caspase-3, IkB-α proteins was determined by histopathologic staining.Methods:The Wistar rats were randomly (male and female) divided to five groups(n=15/group), andthe group include the Con, SCA, EP,VP and V+E groups:(1) The control group (Con group):the rats was only intubated,without apnea and cardiopulmonary resuscitation (CPR);(2) TheSCA group (sudden cardiac arrest): The rats was intubated,apnea and CPR. The saline solution1ml was injected.(3) The EP group(The adrenaline group): The rats was intubated, apnea andCPR. The0.2mg/Kg adrenaline was injected for the resusitation of heart.(4) VPgroup(vasopressin group): The rats was intubated,apnea and CPR. The0.8U/Kg vasopressinwas injected for the resusitation of heart.(5) E+V group(combination adrenaline withvasopressin group): The rats was intubated,apnea and CPR. The0.2mg/kg adrenaline+0.8U/kg vasopressin was injected for the resusitation of heart. The survival ratio of the SCA groupis very low, for the rats of the SCA group was only for CPR, and no drug was injected. The ratsof EP,VP and V+E groups was put to death in1,3,6,12hours. The rat’s heart was kept instained fluid. The heart muscle was observed though the sections of H&E staining, and theexpressions of p-p38, caspase-3, IkB-α proteins was determined by histopathologic stained,and the myocardial apoptosis was detected by TUNEL.Results:1.The ratio of the successful resucition in the EP, VP, E+V group are33.33%,66.67%and80.0%. The spontaneous circulation recovery rate,in the EP group compared with the VPand E+V group was statistically significant (P <0.05),but there was no statistically significant difference in the VP and E+V group (P>0.05).2.The morphological observation in myocardial tissue: The Con group show that themyocardial cells is normal, myocardial fibers arranged neatly, horizontal stripes andcellular structure are clearly, cell membrane is integrity,and there is no cell infiltration,necrosis and fibrosis; The SCA group showed that myocardial fibers partly enrichment,cellular structure confusion, nuclear pyknosis, myocardial cell gap without edema; The EPgroup showed that myocardial cells Irregular, nuclear shrinkage, cellular structure disorder,tissue edema clearly; The VP group and combination group showed that part of the normalmyocardial cells, the myocardial fibers disorder, no obvious edema myocardial cells.3. Immunohistochemistry result: Compared with Con group, the protein expression of p-p38,caspase-3, IkB-a in the SCA group obviously increased (P <0.01); Compared with SCAgroup, the protein expression of p-p38, caspase-3, IkB-a in the EP group decreased (P <0.05), and obviously decreased (P <0.01) in the VP group and E+V group; Comparedwith EP group, the protein expression of p-p38, caspase-3, IkB-a in the E+V groupobviously decreased (P <0.01); there are any difference in the VP group and E+Vgroup.4.TUNEL result: Compared with Con group, the fluorescence intensity was obviouslyenhanced in the SCA group(P <0.01); Compared with SCA group, the fluorescenceintensity was enhanced in the EP group(P <0.05), and obviously enhanced in the VPgroup and E+V group(P <0.01); There are any difference of fluorescence intensity in theVP group and E+Vgroup.Conclusion:Vasopressin combined with adrenaline can obviously increased the rate of spontaneouscirculation recovery, improve the success rate of recovery in the rat’s asphyxia model;Adrenaline can increased myocardial injury, improved cell inflammation reaction and decresedapoptosis; Vasopressin and vasopressin adrenaline can increased the supply of myocardialblood, inflammatory reaction and apoptosis in the rat’s asphyxia model.
