Objective To observe the clinical curative effect of He Ye Jiang Zhi decoction on metabolic syndrome, and the effect of circulating ACE and Ang Ⅱ.Methods Using random Numbers table according to the order,70patients with metabolic syndrome were randomly divided into two groups after they signed informed consent voluntarily.35cases of the control group, treated with routine western medicine;35cases in the treatment group, oral He Ye Jiang Zhi decoction,1dose daily.Both groups were treated for12weeks.Body mass index, blood glucose, insulin resistance index, blood fat, blood pressure, ACE, Ang Ⅱ and TCM clinical symptom score were observed before and after treatment, and recorded the occurrence of adverse reactions. At the same time,20normal subjects served as normal controls from the same medical center to compare the normal levels of ACE and Ang Ⅱ.Results(1) Totally there were60patients completed the12-weeks observation. The number of patients who had completed the observation in the treatment group and the control group was30and30respectively.(2) The level of serum ACE and Ang Ⅱ in MS patients was higher than normal,the difference was statistically significant (P<0.01), indicating that MS patients have the activation of RAS system.(3) WC and BMI of the treatment group and the control group were significantly decreased after treatment,the difference was statistically significant (P<0.01), compared with the control group, BMI of the treatment group decreased significantly(P<0.05), while WC decreased similar to that in the control group, the difference has no statistical significance (P>0.05).(4) SBP and DBP of the treatment group and the control group were significantly decreased after treatment, the difference was statistically significant (P<0.01), compared with the treatment group, the control group SBP and DBP decreased significantly, the difference was statistically significant (P<0.01). The total efficiency of the treatment group was63.3%, the total efficiency of the control group was76.7%, the difference has no statistical significance (P>0.05).The recurrence rate in treatment group was33.3%, the recurrence rate in control group was53.3%, the difference has no statistical significance (P>0.05).(5) FBG,2hPG, FINS, HOMA-IR of the treatment group and the control group were significantly decreased (P<0.01), compared with the control group,the treatment group2hPG,FBG, FINS, HOMA-IR decreased similar to that in the control group, the difference has no statistical significance (P>0.05). The total efficiency of the treatment group was73.3%, the total efficiency of the control group was76.7%, the difference has no statistical significance (P>0.05). The recurrence rate in treatment group was13.3%, lower than the36.7%in the control group, the difference was statistically significant (P<0.05).(6) TC, TG, LDL-c of the treatment group and the control group were significantly decreased after treatment, the difference was statistically significant (P<0.01),.HDL-c of the treatment group was significantly increased, the difference was statistically significant (P<0.01). HDL-c of the control group HDL-c increased slightly, the difference has no statistical significance (P>0.05).Compared with the control group, the treatment group HDL-c increased significantly, the difference was statistically significant (P<0.05), while TC, TG, LDL-c decreased similar to that in the control group, the difference has no statistical significance (P>0.05). The total efficiency of the treatment group was83.3%, the total efficiency of the control group was70%, the difference has no statistical significance (P>0.05). The recurrence rate in treatment group was16.7%, lower than the40%in the control group, the difference was statistically significant (.P<0.05).(7) Serum ACE, Ang Ⅱ of the treatment group and control group were significantly decreased after treatment, the difference was statistically significant (P<0.01). Compared with the control group, serum ACE, Ang Ⅱ decreased similar to that in the control group, the difference has no statistical significance (.P>0.05).(8) The Traditional Chinese medicine clinical symptom score of the treatment group of was significantly decreased after treatment, the difference was statistically significant (P< 0.01),while the Traditional Chinese medicine clinical symptom score of the control group was slightly decreased after treatment, the difference has no statistical significance (P>0.05).Compared with the control group, the Traditional Chinese medicine clinical symptom score decreased significantly, the difference was statistically significant (P<0.01). The total efficiency of the treatment group was90%, higher than the66.7%in control group,the difference was statistically significant (P<0.05).Conclusion The activation of RAS system exists in patients with MS, serum ACE, Ang Ⅱ was higher than normal. He Ye Jiang Zhi decoction was able to loss weight,improve the status of hyperglycemia,hypertension,high cholesterol and insulin resistance and block the activation of RAS systemin in patients with MS.At the same time,it can significantly improve the clinical symptoms in patients with metabolic syndrome.So it is a safe and effective traditional Chinese medicine compound to treat metabolic syndrome.
