Objective:Laryngeal carcinoma is the most common malignant tumor inhead and neck, its incidence has a significant growth trend. The cause oflaryngeal carcinoma is still not very clear, which is the result of a variety ofcarcinogenic factors synergy. Oxidative stress is closely related with thedevelopment of tumor including laryngeal carcinoma. In the process ofoxidative stress, some transcription factors in the signal transmission areactivated, Nrf2and Keap1are the more important transcription factors. Toexplore the expression of Nrf2and Keap1in laryngeal squamous cellcarcinoma and para-carcinoma normal tissues by immunohistochemistrymethod in order to study the expression of Nrf2and Keap1differences inlaryngeal carcinoma and para-carcinoma normal tissues, and to explore therelationships between the expression of Nrf2or Keap1and clinical biologicalbehaviors,such as patient’s age, pathological stage,lymph node metastasis,tumor differentiation and smoking.Further,we investigate the role ofKeap1-Nrf2-ARE pathway in tumor occurrence and development for in orderto provide theoretical basis to laryngeal carcinoma therapy.Methods:35laryngeal carcinoma samples were analyzed and meanwhile35para-carcinoma tissues were also studied as controls.A1l subjects are takenfrom the patients who accepted operation treatment in the Second Hospital ofHeBei Medical University from February2012to September2013.All tumortissues are confirmed to be squamous-cell carcinoma. The patients hadreceived neither chemotherapy nor radiation therapy before operation. Theexpression of Nrf2and Keap1were examined by immunohistochemicaltechnique in tumor tissues of35patients with laryngeal carcinoma and35cases of normal laryngeal squamous epithelium. The relationships between theexpression of Nrf2and Keap1and the clinical factors are analyzed by the SPSS13.0statistical software.Results: The expression of Nrf2in the35cases of laryngeal carcinomawas77.14%, in the para-carcinoma normal tissues was8.57%, the differencebetween the two groups was statistically significant(P<0.01); The expressionof Keap1in the35cases of laryngeal carcinoma was71.43%, in thepara-carcinoma normal tissues was31.43%, the difference between the twogroups was statistically significant(P<0.01).In the35cases of laryngeal carcinoma, the difference between the twodifferent clinical stages of Nrf2has significant(P<0.05), that the expression ofNrf2inⅠ~Ⅱstage was65.00%and the expression of Nrf2in Ⅲ~Ⅳ stage was93.33%; In the35cases of laryngeal carcinoma,the difference between the twodifferent clinical stages of Keap1has significant(P<0.05), that the expressionof Keap1inⅠ~Ⅱstage was65.00%and the expression of Keap1in Ⅲ~Ⅳstage was93.33%In the35cases of laryngeal carcinoma, the positive expression rate ofNrf2in lymph nodes with metastasis was100.00%and the positive expressionof Nrf2in lymph nodes without metastasis was68.00%; In the35cases oflaryngeal carcinoma,the difference with lymph node metastasis of Keap1hassignificant(P<0.05), that the positive expression rate of Keap1in lymph nodeswith metastasis was100.00%and the positive expression of Keap1in lymphnodes without metastasis was60.00%.In the35cases of laryngeal carcinoma, the positive expression of Nrf2and Keap1had no relationship with the elements such as tumordifferentiation,smoking and age (P>0.05).In the35cases of laryngeal carcinoma, The spearman’s Rank CorrelationAnalysis show that the positive expression of Nrf2and Keap1showed nosignificant correlation.Conclusions: The expression of Nrf2in laryngeal squamous cellcarcinoma was higher than that in para-carcinoma tissues. The expression ofNrf2inⅠ~Ⅱstage was lower than that in Ⅲ~Ⅳ stage.the expression of Nrf2in lymph nodes with metastasis was higher than that in lymph nodes without metastasis. The expression of Nrf2was related to the different clinical stagesand lymphatic metastasis, but was not related to pathological differentiationdegree, patient age and smoking.The expression of Keap1in laryngeal squamous cell carcinoma washigher than that in para-carcinoma tissues. The expression of Keap1inⅠ~Ⅱstage was lower than that in Ⅲ~Ⅳ stage. the expression of Keap1inlymph nodes with metastasis was higher than that in lymph nodes withoutmetastasis. The expression of Keap1was related to the different clinical stagesand lymphatic metastasis, but was not related to pathological differentiationdegree, patient age and smoking.Keap1-Nrf2-ARE pathway may play an important role in progression oflaryngeal squamous cell carcinoma.
