Objective: Diabetic neurogenic bladder(DNB) is one of the commonchronic complications of diabetes. It has the characteristics of high incidenceand clinical symptom diversity. It has an impact on long-term prognosis ofpatients. Due to the DNB pathogenesis has not been fully elucidated, thewestern medicine has no effective drug treatment. Traditional Chinesemedicine attaches great importance to the overall adjustment and treatmentbased on syndrome differentiation. Use of traditional Chinese medicinetreatment may work in the onset of multiple link of DNB, so has good curativeeffect. According to the clinical practice and the theory of traditional Chineseand western medicine, we summed up the“Liang Run Tong Luo Fang” for thetreatment of diabetic neuropathy. The purpose of this experiment is to confirmthat “Liang Run Tong Luo Fang” can improve bladder dysfunction by copyingmodel of diabetic neurogenic bladder rats, measuring its urine volume in24hours and bladder wet weight,observing the bladder tissue structure changesunder light microscopy and detecting positive expression of brain-derivedneurotrophic factor (BDNF) of bladder tissue and the level of cAMP inbladder tissue. Another purpose is to explore “Liang Run Tong Luo Fang” thepossible mechanism of improving bladder function.Methods:50male Wistar rats (180-220g) were randomly divided into5groups. There were6Wistar rats were divided as blank control group(Black),11Wistar rats as model group (Moldel),11Wistar rats as westernmedicine group (X),11Wistar rats as medium dose group of Liang run tongluo fang (Z),11Wistar rats as high dose group of Liang run tong luo fang (G)11, after1week in adaptive feed. By injecting streptozotocin according to55mg/Kg body weight, we copied model of diabetic rats after fasting for12hours. No success in rats will be eliminated. We gave a general feed. After1 week, each group rats were gave lavage treatment using respectivelyphysiological saline, cobalt ammonium, middle dose and high dose ofLiangruntongluofang. After treatment12weeks, we observed the test indexes.Results:1In Wistar rats with STZ injected in the50only had2only buildingmodel without success, and3rats died in experiments. So there were5ratsfalling off in experiment.2General condition of rats: compared with the blank control group rats,rats diabetes group generally was in poor condition. After treatment12weeks,the general condition of high dose group of Liang run tong luo fang wasobviously better than other group of diabetes rats.3Blood sugar: compared with blank control group, diabetes group bloodsugar significantly higher (P<0.01) in each time period, and blood sugar hadno obvious difference among groups (P>0.05).4Changes of body weight of groups of rats: compared with blank controlgroup, diabetic rats weight decreased obviously (P<0.01) in every period ofeach group and body weight had no obvious difference among groups(P>0.05).5Changes of urine volume in24hours of rats: after treatment12weeks,compared with blank control group, urine volume in24hours of diabetic ratsincreased significantly (P<0.01). Compared with model group, urine volumein24hours of treatment groups declined (P<0.05). Compared with the westernmedicine group, medium dose group of Liang run tong luo fang had nosignificant changes (P>0.05); high dose group of Liang run tong luo fangdeclined (P<0.05). Compared two Chinese traditional medicine group, highdose group of urine volume in24hours under medium dose group (P<0.05).6Changes of blood lipid: TC, TG, HDL, LDL in blood of each group ratshad no significant difference (P>0.05).7Comparison of bladder wet weight: after treatment12weeks, comparedwith blank control group, bladder wet weight of diabetic rats increased(P<0.05). Compared with model group, bladder wet weight of treatment groups declined significantly (P<0.01). Compared with the western medicinegroup, medium dose group of Liang run tong luo fang had no significantchanges (P>0.05); high dose group of Liang run tong luo fang declined(P<0.05). Compared two Chinese traditional medicine group, high dose groupof bladder wet weight under medium dose group (P<0.05).8cAMP content in bladder tissue: after treatment12weeks, comparedwith blank control group, cAMP content of diabetic rats declined (P<0.05).Compared with model group, cAMP content of western medicine group andmedium dose group of Liang run tong luo fang had no significant changes(P>0.05); high dose group of Liang run tong luo fang increased (P<0.05).Compared with the western medicine group, medium dose group of Liang runtong luo fang had no significant changes (P>0.05); high dose group of Liangrun tong luo fang declined (P<0.05). Compared two Chinese traditionalmedicine group, high dose group of cAMP content above medium dose group(P<0.05).9Observing detrusor structure of rats under the light microscope:compared with the blank control group rats, rats diabetes group generally wasappeared different degree of damage. After treatment12weeks, the damage ofhigh dose group of the urinary bladder was reduced significantly comparedwith other group of diabetes rats.10The results of immunohistochemical staining of BDNF in bladdertissue.10.1Morphological observation: staining of tissue positioned in muscularlayer of bladder tissue. Dyeing place appeared pale yellow, deep yellow to tanparticles of various sizes. Positive particles were present in the cytoplasm andcell nucleus showed negative. Negative result for: no shading. Compared withthe blank control group, dyeing of diabetes groups was shallow and density islow. compared with model group, western medicine and middle dose group ofLiang run tong luo fang was no obvious difference; dyeing of high dose groupof Liang run tong luo fang was depth and density was high.10.2The results accumulated optical density: compared with blank control group, the accumulated optical density of diabetes groups decreasedsignificantly (P<0.01). Compared with model group, optical density ofwestern medicine group and medium dose group of Liang run tong luo fangwas not significantly change (P>0.05);optical density of high dose group ofLiang run tong luo fang were higher(P<0.05). Compared with the westernmedicine group, optical density of medium dose group was not significantlychange (P>0.05) and optical density value of high dose group increasedobviously (P<0.01). Compared to two Chinese traditional medicine groups,optical density of high dose group increased significantly (P<0.01).Conclusions:1Liang run tong luo fang can significantly improve general status,reduce urine volume in24hours and bladder wet weight and improve thestructure of the urinary bladder of diabetic rats. It explains that Liang run tongluo fang can prevent and treat the DNB and its efficacy is superior to theammonium methyl cobalt.2Mchanism of Liang run tong luo fang improving bladder function maybe by improving expression of BDNF and content of cAMP. This experimentprovided a reliable basis for the clinical application.
