Objective:Brain arteriovenous malformation is a spontaneous intracranialvascular diseases.Due to local vascular dysplasia, and it forms of vascularmalformation which is tortuous and unequal thickness. The study founds thattyrosine kinase family have EphrinB2ligand/EphB4receptor bidirectionalsignaling pathway which can promote blood vessel development, plays animportant role in the process of promoting the migration, proliferation anddifferentiation by vascular endothelial cell.And controls the regulation fromthe formation and development of arteriovenous malformation vascular massanomaly. In the long-term high hemodynamics of cerebral arteriovenousmalformation abnormal vascular group, it will lead to the existence of hypoxiaenvironment relative to the surrounding tissue, stimulate the organization tosecreting growth factors to promoting vascular to forming and remodeling.Expression of EphrinB2ligand and the change of hemodynamics will regulatevascular development, which lead to the formation of abnormal brainarteriovenous malformation vascular mass. The interaction mechanismbetween EphrinB2ligand and hemodynamics is not clear. Study the change ofEphrinB2ligand’s expression level with different hemodynamic state of thebrain, to explore the correlation between the expression of EphrinB2ligandand the state of hemodynamics, to explore the change of micro-environmentfrom different hemodynamic state how to influence the expression ofEphrinB2ligand and develope the vascular. It can provide theoretical andexperimental basis for the clinical diagnosis and treatment of Brainarteriovenous malformation。Methods: With the help of digital subtraction angiography (DSA), group48patients with cerebral venous malformation into two groups, according to thepartition interval (A-V),which comes from the blood through artery to vein,that express the state of hemodynamics.The two groups are: high outputand low resistance (A-V <2seconds),20cases and low output and highresistance (A-V>2seconds),28cases. To obtain each paraffin embeddedspecimens comes from the patients with brain arteriovenous malformation andoperate them by immunohistochemical technique.The positive results is theexpression of EphrinB2ligands, group them into strong staining group andweak staining groups, and then analysis the correlation of the expression levelof EphrinB2ligand and the state of hemodynamics by the SPSS16.0.Results: The expression of EphrinB2ligand is in the artery endothelialcells,find that48cases of brain arteriovenous malformation is weak staininggroup26cases and strong staining group22cases. Combined with the stateof hemodynamic,48patients will be divided into four groups: high outputand low resistance, strong staining18cases, high output and low resistance,weak staining2cases; low output and high resistance, strong staining4cases,low output and high resistance,weak staining24cases. The correlationanalysis results by SPSS16.0is P<0.01, considered that the expression levelof EphrinB2ligand and the state of hemodynamic have relevance, thecorrelation coefficient is C=0.6.Discussions:Experimental studies in recent years found: EphrinB2ligand/EphB4receptor bidirectional signal transduction pathway plays an importantrole in the process of brain arteriovenous malformation. Animal experimentsshowed that EphrinB2ligand can promote the development of blood vessel,lack of EphrinB2ligand gene in mouse will lead to mouse embryonic deathdue to the defects of vascular development. New blood vessels in tumor tissuefind that the expression level of EphrinB2ligand increasing. Vascularendothelial cell culture experiments show that, if the medium containsEphrinB2-Fc, it can promote the proliferation of vascular endothelial cells,promote vascular regeneration.Epidemiological survey find: Due to Doppler ultrasound (TCD) detection, theresistance index(RI) of brain arteriovenous malformation less than normalintracranial artery, the hemodynamics of brain arteriovenous malformation mass than normal intracranial artery,it will lead malformation vascular massin hyperdynamic state for a long time.At present, the digital subtraction angiography (DSA) is the“gold standard”inthe diagnosis of brain arteriovenous malformation.It not only can find thevascular mass size,shape and the adjacent relationship, also can clear theartery and vein. At the same time, it can calculate the time that blood throughartery to vein (A-V) by the per frame image,and show the state ofhemodynamics.The results showed that the state of hemodynamic and the expression ofEphrinB2ligand have association, and the correlation coefficient is0.6,Indicating that the change between hemodynamics and EphrinB2ligand willpromote the development of brain arteriovenous malformation. High status ofhemodynamic can stimulate the expression of EphrinB2ligand, and controlthe proliferation, migration and differentiation of vascular endothelialcell,promote the development of brain arteriovenous malformation.Conversely, reduce the status of hemodynamic will reduce the expression ofEphrinB2ligand, inhibition the development of brain arteriovenousmalformation. Further study on the mechanism of the interaction between thehemodynamic and EphrinB2ligand, will provide theoretical and experimentalbasis for clinical treatment by changing the hemodynamic status.
