ObjectivePrimary glomerulopathy(PGN) is the main reason of chronic renal failure.Renal interstitial fibrosis(RIF) was the key progress when PGN leading tochronic renal failure. Recent research indicated that heme oxygenase-1(HO-1)may be a protective factor to kindey,through inhibiting or even reversing RIF. Toinvestigate the effects of HO-1on the development of PGN and provide a strongbasis for the prevention and treatment of renal interstitial fibrosis.MethodsWe chosen33cases inpatients (20males and13females) withnephropathy from October2012to January2013who pathological changeswere identified by renal biopsy(nephropathy group), including chronicglomerulonephritis17cases, nephrotic syndrome14cases, Insidious glomerularnephritis2cases. In the group,there were20male and13female, the age ofpatients was from17to61years, mean38.73±15.06years. Observed andrecorded clinical parameters including systolic pressure(130±10.45mmHg),diastolic pressure(95±9.72mmHg),mean arterial pressure(103.45±11.80mmHg),serum creatinine(128.21±26.25umol/L), urea nitrogen (128.21±26.25umol/L), blood uric acid (331.39±64.74umol/L),urine protein of24-hour(1.86±1.19g/24h),ect,and the glomerular filtration rate(GFR) estimatedby the simplified MDRD formula. According to the World Health Organization(WHO1995) kidney disease histological classification criteria,33patients renal biopsy pathologic types were typed.The pathological parameters of the tubularsemi-quantified integration reference Katafuchi scoring standard,and accordingtubulointerstitial integral(TI),the33cases were divided into four groups: group A(0score); group B (1-3scores); group C (4-6scores)and group D (7-9scores).The expression levels of HO-1in renal tissue and urinary of PGNpatients were detected respectively by indirect immunofluorescence stainingand ELISA double antibody sandwich method.20healthy cases (13males and7females) as a control which included the renal tissue control of6cases nomalrenal tissue far away from Renal neoplasm tissue. Detected renal tissue and theurinary sediment HO-1expression levels,pathology and clinical data werestatistically analyzed.Results1. HO-1expressed of different degrees in renal tubules of nephropathygroup,no immunofluorescence expressed in kidney glomerulus. In the normalcontrol group,there were only2cases weakly expressed in the renal tubules.Overall expression level was significantly higher than the normal control group,the difference was statistically significant (P<0.05).2. With the aggravation of renal interstitial fibrosis,the expression of HO-1decreased gradually in renal interstitium of nephropathy group,and theImmunofluorescence intensity was negatively correlatedwith TI (r=-0.436,P=0.043).3. The expression level of HO-1in the urinary of nephropathy group,was220.05±27.73ng/L,and108.97±55.27ng/L in the normal control group, Theexpression level of HO-1in the urinary of nephropathy group was significantlyhigher than the control group,the difference was significant(P<0.01).4. The expression level of HO-1in the urinary was173.60±49.65ng/L ingroup A,192.43±36.76ng/L in group B,230.09±52.99ng/L in group C and256.40±97.27ng/L in group D.The difference in the four groups was statistically significant(P=0.028).5.The expression level of HO-1in the urinary of nephropathy group waspositively correlated with the degree of the renal interstitium injury.(r=0.335,P=0.041).Conclusions1. HO-1was highly expressed in various degrees in renal interstitium ofnephropathy group,the main position was renal tubules,but the degree ofexprssion was declined with the renal interstitial fibrosis aggravation.2. The expression of HO-1in the urinary of nephropathy group wassignificantly higher than the normal control group,and the expression levelincreased with the aggravation of renal interstitial fibrosis.3. HO-1was a kind of stress protein from the process of Cellular oxidativestress. the detection of the expression of HO-1in renal interstitium andurinary maybe a indicators of reflection the extent of renal interstitial damage.
