ObjectivesThe importance of oral care in mechanically ventilated patients has beenwidely recognized.Many studies have shown that oral health status has adirected relation with ventilator-associated pneumonia (Ventilator-AssociatedPneumonia VAP).Over the years,people have done many studies of oral caresolution, but so far still all have our say.In this paper,distilled water,0.2%chlorhexidine,and the NS were used in clinical to reduce oral infection,reducethe incidence of VAP,and provide better oral care solution for clinical basis.MethodsIn this study,single-blind randomized controlled study was used in JinzhouCentral Hospital in Liaoning Province with intubation mechanical ventilation150patients in ICU(Intensive Care Unit ICU).The patients meeting the conditions oftest were divided into three groups (group A distilled water, group B0.2%chlorhexidine,group C NS),47cases of group A,47cases of group B and43cases of group C, using oral irrigator with cotton scrub method. All patients dothe oral care three times daily,respectively8am,2pm,8pm,carried out for sevendays then compared three groups of patients with oral Ph value, mouth ulcersand VAP occurrence.Oral ulcers and VAP occurrence compared by x~2test.ThePh value of three sets of oral care before and after use the t test,the groupswere compared using one-way ANOVA.ResultsDrawn by the trial, the incidence of VAP in patients with distilled water group lower than the NS group, the difference was statistically significant(P<0.05),distilled water group and0.2%chlorhexidine group,0.2%chlorhexidinegroup and the NS group,the difference was not statistically significant (P>0.05);Incidence of oral ulcers,distilled water group is better than the NS group,the difference was statistically significant (P<0.05),other groups was notstatistically significant (P>0.05); The oral Ph values, the difference between thethree groups was statistically significant (P<0.05),distilled water group and0.2%chlorhexidine group and saline group,there was statistically significant(P<0.05),There was no statistically significant between0.2%chlorhexidinegroup and the NS group (P>0.05).Oral care before and after the Ph value bypaired t-test, only distilled water group difference was statistically significant(P<0.05).ConclusionsObtained in this study,distilled water can correct the patient’s mouth Ph value;better than NS in reducing oral ulcers and the incidence of VAP,Though therewas no difference with0.2%chlorhexidine,but the distilled water can reducecosts,low prices and do not cause flora compared with0.2%chlorhexidine, canbe widely promoted in clinical.
