Objective:To study the effect of angiotensinogen (AGT) and insulin-like growthfactor-1(IGF-1) on the progress of diabetic nephropathy.Methods:46cases were in line with WHO (1999) diabetes diagnosis standardand32cases recently in the healthy check-up were selected in the Department ofNephrology and Endocrinology in First People’s hospital from March2012to July2012.All of them except tumors,tuberculosis, liver damage, DM enough, otherendocrine and metabolic diseases, surgery, trauma, or other stress conditions and theexclusion of the patients taking angiotensin-converting enzyme inhibitors (ACEI) and/or angiotensinu-II receptor antagonist (ARB) in the last two months.According torenal lesions,the subjects were divided into early diabetic nephropathy group (20cases) and clinical diabetic nephropathy group (26cases).Serum and urine AGT, IGF1levels were measured with ELISA in78subjects.New configuration streptozocinwas fasting single sterile intraperitoneal injected to establish the rats model of diabeticnephropathy,and using ELISA method to detect serum and urine AGT, IGF-1levels ofdifferent periods(4week、12week)rats, and the AGT, IGF-1protein expression inkidneys were assessed by immunohistochemistry and computer aided video system.Results:1、Plasma AGT、IGF-1levels in the Early diabetic nephropathy groupand clinical diabetic nephropathy group were significantly higher than the normalcontrol group,differences exist statistical significance(P<0.05).Levels in the EDN and CDN group were significantly higher than the NC group,besides,along with theprogress of the disease increased significantly,differences exist statisticalsignificance(P<0.05).2、Along with progress of disease,model group rats expressedmore urinary albumin excretion,and glomerular volume increase early,decreaseobviously later, glomerular structure was obviously damaged.The increase of serumAGT、urinary AGT、IGF-1and the decrease of serum IGF-1were observed during thewhole experiment,differences exist statistical significance(P<0.05).AGT and IGF-1proteins were expressed mainly in renal tubule of model and control group.The levelof AGT and IGF-1proteins in the model group increased remarkably comparing withcontrol group assessed by immunohistochemistry(P<0.05).Conclusion:AGT and IGF-1involved in the pathogenesis of diabeticnephropathy, the change of AGT and IGF-1may promote the development of diabeticnephropathy, therefore,restrain the expression of AGT and IGF-1protein,may providea new way for prevent and control of DN.
