BackgroundChina is a country with the highest incidence and mortality of esophagealcancer(esophageal cancer, EC) whose mortality rate ranks second of the nationalcancer mortality. The main features of esophageal cancer prognosis: up to90%ofpatients with early esophageal cancer surgery survived after5years,while inpatients with advanced the5-year survival rate is only10%. Therefore,the bestchoice to lower esophageal cancer mortality, improve long-term survival of patientswith esophageal cancer is to diagnosis and treatment it earlier. Tumor markers (tumormarker, TM) is the most commonly tools used in clinical of early tumor diseasescreening. Though there has grown to hundreds of types of tumor markers, no oneparticularly desirable markers can be used clinically in screening early esophagealcancer, prompting transfer and indicating prognostic. Osteopontin (osteopontin, OPN)is a phosphorylated glycoprotein with variety biological functions found in recentyears, many studies have shown that tumor occurrence, development and metastasis,especially bone metastasis are intimately related to OPN. Some studies done by somescholars shows the potential value of OPN as a new tumor marker though analyingserum or plasma OPN levels of patients with colon cancer, breast cancer, pancreaticcancer, ovarian cancer, lung cancer, liver cancer and other tumors.There are also studies have confirmed that osteopontin in patients with esophageal cancer tumortissue are significantly higher than that of normal tissue, which is intimetly related toTNM stage, lymph node metastasis, and prognosis. But now there is still no clearconclusion of serum OPN in patients with esophageal cancer.Now more and moreattentions were pay to the study of clinical application of OPN as a potential tumormarkers in esophageal cancer.ObjictiveIn this study, we aimed to investigate the role and clinical value of OPN inesophageal cancer early diagnosis, predicting transfer,by the determinating theserum OPN of normal human,and serum OPN, CEA, Cyfra21-1of esophagealcancer patients before surgery, and also postoperative serum OPN of esophagealcancer patients.Analying the relationship between preoperative serum OPN levels ofesophageal cancer patients and serum OPN levels in normal, postoperative serumOPN and CEA,Cyfra21-1of esophageal cancer patients. Analying the relationshipbetween preoperative serum OPN levels and gender, age, lesion, clinical stage, Tstaging, N staging and so on.Patients And Methods Background121patients were chose diagnosed as esophageal cancer and without mergehypertension and (or) diabetes. All patients had not received radiotherapy andchemotherapy.40healthy blood donors as a control group. Detected early morningfasting peripheral venous blood of the control group bye the method of theenzyme-linked immunosorbent assay (ELISA) skill, and also early morning fastingperipheral venous blood OPN levels of esophageal cancer patients of a week beforeand after surgery. Use the method of eletro-chemiluminescenceimmunoassay(ECLIA) detected the fasting blood serum CEA, Cyfra21-1ofesophageal cancer patients a week before surgery. Collected and analysis patients,pathology and clinical data. Results1. Esophageal cancer patients with preoperative serum OPN levels higher thannormal serum OPN levels (P <0.05).2. Esophageal cancer patients serum OPN decrease statistically significant (P<0.05), but still higher than normal.3. Early esophageal cancer patients preoperative serum OPN levels lower than inpatients with advanced preoperative serum OPN levels (P <0.05).4. Esophageal cancer patients with preoperative serum OPN levels, gender, age,lesions, pathological type correlation (P>0.05), significantly correlated withlymph node metastasis (p <0.001).5. Joint detection of esophageal cancer patients with preoperative serum OPN leveland serum CEA Cyfra21-1can improve the sensitivity of early diagnosis ofesophageal cancer.Conclusions1. The serum OPN may use as the tumor marker to evaluate of esophageal cancer‘soccurrence, development, and transfer of certain indicators.2. It can be improved the accuracy of the EC diagnosis of the serum OPNcombination with other tumor markers...
