Objective:This study is to investigate the relationship of coronary artery lession and risk factors with ischemic cardiomyopathy.Methods:A total of557consesutive patients who had been adimted in the cardiovascular department of XiangYa Hospital and were diagnosed coronary heart disease (excluding patients with acute and subacute myocardial infarction) by coronary angiography from May,2008to March,2013were studied. According the results of coronary arteriography(CAG) and ultrasonic cardiogram(UCG) after admission,all of the patients were divided into two groups:ischemic cardiomyopathy (ICM) group(n=275)(including213males(77.7%) and62females (22.3%), average age (63.35±7.165)year-old) and non-ischemic cardiomyopathy(non-ICM) group(n=282)(including186males and96females,average age (61.75±8.59)year-old).Basic information, medical history,results of CAG and laboratory tests of all the patients were analized.Results:(1) Age, history of myocardial infarction and diabetes mellitus were positively correlated with cardiac dilatation in CHD(p<0.05). (2)HDL-C and apoA1in ICM group were much lower than the other group(P<0.05), and FFA,hsCRP and UA in ICM group were higher than in the non-ICM group(P<0.05).(3)Gensini score,the number of branch and diffuse lesion of coronary artery were positively correlated with cardiac dilatation in CHD(p<0.05),and collateral circulation of coronary artery was negative correlation with it(p<0.05).(4) The expansion degree of left ventricular in ICM group was positively correlated with diabetes mellitus(p<0.05),and were negative correlated with age.smoking, history of myocardial infarction and hypertension(P>0.05).(5) The expansion degree of left ventricular in ICM group had no relationship with all laboratory tests results(P>0.05).(6) The expansion degree of left ventricular in ICM group had relationship with Gensini score(P<0.05),and had no relationship with the number of branch and diffuse lesion of coronary artery(P>0.05). The expansion degree of left ventricular in ICM was negative correlation with collateral circulation(P<0.05).Conclusion:(1).There are many influences factors on the left ventricle enlargement in ischemic cardiomyopathy,of which Age, history of myocardial infarction,diabetes mellitus and the degree and characteristic of coronary artery stenosis are the leading causes. (2).Diabetes mellitus can aggravate the expansion degree of left ventricular in ICM.(?).Collateral circulation can delay the process of the left ventricular expansion in ICM.
