| ObjectiveTo observe the effect of applying heterogeneous (cattle) acellular dermal matrix for pharygolanryngeal defect repaire after pharygolanryngeal carcinoma resection,investigate the feasibility of heterogeneous (cattle) acellular dermal matrix in pharygolanryngeal defect repaire after pharygolanryngeal carcinoma resection.MethodWe studied30patients with pharygolanryngeal carcinoma who underwent pharygolanryngeal carcinoma resection in the Department of Otolaryngology-head and neck surgery,the second xiangya hospital of Central South University, form March2011to February2013.Pathology of cancer was all epidermoid carcinoma.Tumor growth areas was all in the pharygolanryngeal lientang wall.16cases in test group were repaired with heterogeneous (cattle) acellular dermal matrix,14cases in the control group were repaired with pectoralis major myocutaneous flap,we observed the change of repaired area through the electronic laryngoscope after operation and counted up respectively operation time, intraoperative blood loss, total flow rate, postoperative recovery time, swallowing function recovery time, incidence of pharyngeal fistula and infection rate of incision.ResultsAll repair survived, repair area in test group were good, no obvious scar. In test group operation time, intraoperative blood loss, total flow rate, postoperative recovery time, swallowing function recovery time, incidence of pharyngeal fistula, infection rate of incision were4.41±0.44h,358.13±50.36ml,448.13±72.32mlã€17.06±4.73dã€16.19±2.51dã€6.25%(1/16cases)ã€6.25%(1/16cases); in control group operation time, intraoperative blood loss, total flow rate, postoperative recovery time, swallowing function recovery time, incidence of pharyngeal fistula, infection rate of incision were5.42±0.43hã€478.50±42.94mã€658.57±100.83mã€20.29±3.34dã€24.14±3.37dã€7.14%(1/14cases)ã€14.29%(1/14cases).Comparative analysis of test group and control group,in operation time, intraoperative blood loss, total flow rate, postoperative recovery time, swallowing function recovery time, the differences were statistically significant, the incidence of pharyngeal fistula and incision infection rate had no statistical significance. ConclusionsHeterogeneous (cattle) acellular dermal matrix is a new method to repair defect of pharygolanryngeal lientang wall after pharygolanryngeal carcinoma resection.The operative procedure is easy to perform and obtains good effect; compared with the pectoralis major myocutaneous flap to repair,it makes postoperative recovery time shorter, recovery of swallowing function quicker and surgical trauma small. |
