Abstract/Summary:
Objective1.To access the left ventricular systolic function at acute stage of Kawasaki disease in children with traditional index using common echocardiography.2. To evaluate the left ventricular systolic function at acute stage of Kawasaki disease in children using velocity vector imaging.3、To explore the value of velocity vector imaging technique on left ventricular systolic function of acute stage of Kawasaki disease in children.Methods32cases of acute Kawasaki disease who were diagnosed in the second Xiangya Hospital were collected, All the patients were in line with the standard which was made by Japan Kawasaki disease Committee in1984.According to coronary artery diameter,32cases of acute Kawasaki disease were divided into two groups:no coronary artery dilatation group and coronary dilation group.30normal children were enrolled as controls. left ventricular ejection fraction (EF)and left ventricular fraction shortening (FS) and stroke volume (SV) were measured routinely by common echocardiography, Drawing peak velocity (Vp), strain (S), strain rate (SR) curve diagram by VVI, to measure the longitudinal systolic peak velocity (Vp), strain (S), strain rate (SR)of different segement in left ventricular.Results1、Taditional index by common echocardiography:1) All of the parameters containing EF, FS, SV have no significant difference among three groups.2、The characters of curve diagram of VVI: 1) Velocity(Vp) curve diagram:The shapes of coronary dilation group and no coronary artery dilatation group are similar, which are composed with one positive wave of systole and one negative wave of diastole, but the normal children Vp curve diagram are composed with one positive wave of systole and two negative wave of diastole.2) Strain rate (SR) curve diagram:The shapes of coronary dilation group and no coronary artery dilatation group are similar, which are composed with one negative wave of systole and one positive wave of diastole, the normal children SR curve diagram are composed with one negative wave of systole and two positive wave of diastole.3) Strain (S) curve diagram:The shapes are similar among three groups, the flat-tops of early diastole are not obvious between coronary dilation group and no coronary artery dilatation group.3、datas of VVI:1) The Vp were decreased from basal segments, middle segments to apex segments by turns among three groups.2) The Vp, S, SR in both dilation group and no coronary artery dilatation group were lower than normal control group (P<0.05).3) The Vp, S, SR in dilation group were lower than those of no coronary artery dilatation group in basal segments and middle segments (P<0.05).Conclusions:The left ventricular longitudinal systolic function in acute Kawasaki disease children was lesioned, which can be measured exactly by VVI.
