Objective To study the relationship between the cervical vertebra physiology angle、posterior shift of spinal cord and clinical curative after posterior decompression on the patients with cervicalspondylotic myelopathy.To evaluate the clinical outcome and safety in the patients in whom expansive open-door laminoplasty.To provide an objective basis for clinical application.Method Results in30patients who underwent expansive open-door laminoplasty for cervical spondylotic myelopathy were investigated.Male are22cases and female are8cases.The average age is61.1±10.83years old.30patients were devided in3groups depended on cervical vertebra physiology angle.The preoperative and postoperative6months spinal cord function was evaluated using Japanese orthopaedic society (JOA) rating criteria.Using MRI imaging on central sagittal T2-weighted images to measure the degree of spinal cord drift.Use statistical software to study the relationship between the above three groups of data.Results There wre relationship between the cervical vertebra physiology angle、posterior shift of spinal cord and clinical curative after posterior decompression on the patients with cervicalspondylotic myelopathy.The clinical curative difference between A、B、C group was statistically significant,p<0.05.The shift of spinal cord difference cord between3groups was statistically significant,p<0.05.There was significant correlation between the shift of spinal cord and the cervical vertebra physiology angle, r=0.793,p<0.05.The clinical curative difference between the group, the shift of spinal cord less than3mm,and the group, the shift of spinal cord more than3mm,was statistically significant,p<0.05.Conclusion There were relationship between the cervical vertebra physiology angle、posterior shift of spinal cord and clinical curative after posterior decompression on the patients with cervicalspondylotic myelopathy.The expansive open-door laminoplasty is safe and effective.The cervical vertebra physiology angle was the affeeting faetor of shift of spinal cord and clinical curative.When the spine angle was in the range4.3°-19.6°, clinical curative and the shift of spinal cord were better as spine angle was bigger. If the shift of spinal cord was less than3mm,the clinical curative would be bad.When the pressure thing was bigger than4.9mm,we should use the anterior surgery.
