Objective:To explore the value of real-time three-dimensional echocardiography (RT-3DE)in the evaluation of left and right ventricular systolic function in patients with dilatedcardiomyopathy(DCM) and analysis the correlation of left and right ventricularsystolic function.Methods:Thirty five patients with DCM and forty healthy individuals were enrolled,byusing the contrast detection of the two dimensional echocardiography (2DE),tissuedoppler imaging(TDI)and RT-3DE with Philips IE33color doppler ultrasonicdiagnostic apparatus. with a plain face Simpson method measured2DE indicators:leftand right ventricular systolic function parameters, including end-diastolic volume(EDV), end-systolic volume (ESV), ejection fraction (EF), Measured TDI mitralvalve and tricuspid valve indicators: the interval and lateral wall peak motionvelocities at systole phase(Vs) and then calculating the average of bothVs,usingQLAB8.0quantitative analysis software measured RT-3DE indicators:Left and right ventricular systolic function parameters, including ESV, EDV, EF,examined the possible correlation of left ventricular and right ventricular functions.Results:1. Left and right ventricular systolic function parameters comparison: The resultsof EDV and ESV were significantly higher in patient group with DCM than those inthe control group(P<0.01). The result of EF and the Vs of mitral valve and tricuspidvalve was signicantly lower in patients with DCM than in the control group (P<0.01),2. The result of RT-3DE compared with2DE in patients with DCM, left andright ventricular systolic function parameters LVEF, RVEF has a good correlationbetween RT-3DE and2DE(LVEF: r=0.66, P<0.01, RVEF: r=0.64. P<0.01),but theresult of LVEDV, LVESV, RVEDV, RVESV measured by2DE were significantlylower than RT-3DE measured,the result of LVEF, RVEF measured by2DE weresignificantly higher than RT-3DE measured, and there was statistically significant difference between the two methods(P<0.01).3.The correlation analysis results of left and right ventricular systolic functionparameters of RT-3DE and TDI in patients with DCM: left ventricular function EF,TDI mitral valve indicators the interval and lateral wall peak motion velocities atsystole phase(Vs), the average of both Vs had a positive relation(r1=0.59, r2=0.73,r3=0.73, P<0.01), right ventricular function EF, TDI tricuspid valve indicators theinterval and lateral wall peak motion velocities at systole phase(Vs), the average ofboth Vs had a positive relation(r1=0.63, r2=0.88, r3=0.85, P<0.01),4. The correlation analysis results of left ventricular and right ventricular systolicfunction in patients with DCM: the result of LVEFand RVEF measured by RT-3DEare positively corrected (r=0.59, P<0.01).Conclusions:1. RT-3DE can accurately assess of ventricular systolic function in patients withDCM by measuring the left and right ventricular systolic function parameters.2. Left ventricular systolic function had a positive relation with right ventricularfunction in patients with DCM and indicate that not only appear left ventricularsystolic function obstacle, also appeared different degree of right ventricular systolicfunction obstacle.
