ObjectiveEvaluate the left atrial function of patients with renal failure by the SRI.MethodsFrom February2012to February2014, we chosed50patients with renal failure and20normal controls to check heart by the echocardiography. Firstly theelectrocardiogram (ecg) was connected, and after displaying the ecg waveform on thesynchronous screen, the systolic inner diameter of LA was measured from left ventriclemajor axis view in a two-dimensional model. Three images of the cardiac cyclegathered from the apical four chamber view were deposited in the instrument. Thenstarted the TVI mode, and three dynamic images of the cardiac cycle from the apicalfour chamber view, two chamber view and three chamber view were saved in case ofthe offline analysis. The offline analysis adopted the method of simpson, and measuredthe left atrial maximum volume (LAmax) and the minimum volume (LAmin), and theleft atrial P volume(pla), the left atrium ejection fraction(LAEF). Then used the analysissoftware Q-lab, in which the sampling volume was0.2x0.3cm and the sampling positionwas1.0cm above the left heart cross, to measure the strain rate(SR) in the atrialseptum, the side wall, the antetheca, the inferior wall and the back wall of left atrium,and the peak velocity of SRe, SRa, Srs, then recorded.Results1. Conventional echocardiography: the left atrial volume of the renal failure groupincreased significantly. The left atrial systolic function of the majority of patients(43of50patients, accounting for86%) was greater than or equal to50%, and only a smallnumber of patients’(7patients, accounting for14%)was less than50%.2. Strain rate imaging: The contrast between the renal failure group and the controlgroup shows that the strain rate peak, SRe and SRs, of the renal failure group wassignificantly reduced compared with the control group(the P value was less than1). It indicated the damage for the vessel function and the reservoir function of atrium. Butthe strain rate peak, SRa, of the two groups had no significant difference(the P valuewas greater than0.05). This indicated that the left atrial pump function of the renalfailure group was not effected.3. The strain rate, SRe/a, of renal failure group was less than1, but the control group’swas greater than1. Especially, the inferior wall, the antetheca and the back wall weremost remarkable(the P value was less than1).4. The renal failure group was divided into two classes according to the age of50, andtheir strain rate, SRe,SRa,SRs, had no significant difference(the P value was greaterthan0.05).ConclusionPatients with renal failure can lead to the abnormal function of left atrium, including thevessel function, the reservoir function,; SR can quantificationally detect the strain rateof local myocardium of left atrium accurately and fast. The evaluation of atrial functionby the indexes for strain rate can provide important basis for the clinical treatment.
