Objective:Mature cystic teratoma(MCT) of the ovary,also known as dermoid cystis a kind of benign ovarian tumor, derived from primordial germ cells by multiplemesodermal tissue structure, or even with a mesoderm, mostly unilateral, bilateral10%-17%. At present, the diagnosis of MCT depends mainly on iconography, due to ovarianteratoma ingredients contained different proportions, so the sonographicfeature is varied,complex, or difficult to be differentiated from the other tumors of the ovary, prone tomisdiagnosis, misdiagnosed. It requires additional laboratory examination to assess thediagnosis of MCT. In recent years, research at home and abroad have been reported: theserum CA19-9level increased significantly in MCT, can be considered as a clinicaldiagnostic reference of MCT. The purpose of this study is to explore the value of serumCA19-9,CA125, CEA, AFP in the diagnosis of MCT, more about whether serum CA19-9has value and great significance on clinical diagnosis of MCT.Methods:This retrospective study was performed in127patients with MCT of theovary, who were treated by surgery in the First Affiliated Hospital of Dalian MedicalUniversity between January2011and December2012. All patients underwenttransabdominal or transvaginalultrasound examinations before surgery to measure thediameters of their tumors and to detect any bilaterality. The final decision was on routof pathology. The three tumor markers analyzed in addition to CA19-9were CA125, CEAand AFP. All patients had blood samples preoperatively. All patients had no hepatitis,gastrointestinaldiseases, other malignant tumor and endometriosis. Patients withelevated CA19-9group were57cases and normal CA19-9group were70cases; whileelevated CA125group10cases and normal CA125group117cases; elevated CEAgroup2cases and none had AFP elevated. Statistical analysis was performed withSPSS13.0software. The data was all expressed as mean±standard deviation(x±s).Counting material was analyzed by t test or Mann-Whitney rank sum test, and multiplesamples were analyzed by the Kruskal-Wallis rank sum test,difference in masculine rata was analyzed by χ2test. P <0.05there is statistical significance.Results:1. The average level of all patients with serum CA19-9(52.24±82.24) u/ml; serum CA125(19.18±17.93) u/ml; serum CEA (1.59±1.40) ng/ml; serum AFP(1.82±1.02) IU/ml.2. The positive rate of serum CA19-9in patients was44.88%(57/127). The positiverate of serum CA125was7.87%(10/127). Serum CEA positive rate was1.57%(2/127).Serum AFP positive rate was0%(0/127). The positive rate of CA19-9in ovarian matureteratoma patients was the highest.3. Elevated CA19-9group, the average age (32.56±12.14) years old, CA19-9normalgroup, the average age (35.40±12.93) years old, compared to two age group, nosignificant difference,P>0.05. Elevated CA19-9group, the mean tumor diameter (7.75±2.89) cm, CA19-9normal group, the mean tumor diameter (6.4±2.63) cm, comparedwith the tumor size of two groups, the difference was statistically significant, P<0.05.4. Elevated CA125group, the average age (34.60±11.72) years old, CA125normalgroup, the average age (34.09±12.73) years old, compared to two age group, nosignificant difference,P>0.05. Elevated CA125group, the mean tumor diameter (10.4±4.25) cm, CA125normal group, the mean tumor diameter (6.72±2.48) cm, comparedwith the tumor size of two groups, no significant difference.5. Elevated CA19-9group and normal CA19-9group tumor bilateral rates were17.54%,5.71%, compared with the bilateral occurrence rates of two groups, thedifference was statistically significant, P<0.05; and elevated CA125group and normalCA125group tumor bilateral occurrence rates were40%,8.55%, compared with thebilateral occurrence rates of two groups, the difference was statistically significant,P<0.05.Conclusion:1. The serum level of elevated CA19-9is an important indicator thatcontribute to the diagnosis of MCT.2. The serum level of elevated CA19-9and (or)CA125can be served as a warningsignal of MCT for bilateral, and used to identify the unilaterality and bilaterality ofMCT, guiding operation.3. According to the elevated level of the serum CA19-9, we could preliminary judgethe size of the MCT.4. CEA, AFP diagnosis of ovarian mature teratoma were meaningless and clinicalcan not use them.
