Objective: to evaluate the curative effect and safety of wenxin granule in thetreatment of frequent atrial premature beat (APB) and ventricular premature beat(VPB),and to observe the influence of the corrected QT dispersion (QTcd), heart ratevariability (HRV) and the24-hours mean heart rate.Method:1、60in-patients with APB and60in-patients with VPB from departmentof Cardiology of Liaoning province people’s hospital (from December2011toDecember2012)were selected. The cases of APB were randomly divided into thetreatment group (n=30) and the control group (n=30).The cases of VPB wererandomly divided into the treatment group (n=30) and the control group (n=30).Allpatients meet the diagnostic criteria of APB and VPB in the filed of western medicine.All patients were treated according to specific conditions of the original basis fordisease treatment. There were no significant difference in sex, age and basic diseasesbetween treatment group and control group respectively (P>0.05). The treatment groupof APB and VPB were given wenxin granule1package (18g)3times daily, drink itwith warm boiled water,4weeks for1course of treatment. Control group were given aplacebo1package (18g)3times daily,4weeks for1course of treatment.2、 Clinical symptoms before and after receiving medication, efficiency,effectiveness and total effective rate were observed respectively. Statistical processingwas performed, and the curative effect was observed and compared.3、Before and after receiving medication, electrocardiogram (ECG) and24-hoursdynamic electrocardiogram (Holter ECG monitoring) were done. The change of thenumber of premature beat, corrected QT dispersion (QTcd), heart rate variability (HRV) and the24-hours average heart rate were observed.4、The laboratory examination of blood routine, urine routine, stool routine, liverand kidney function and electrolyte were done and taken as safety indicators to judgeadverse reactions and toxic and side effects.5、Do data processing using spss17.0statistical software. The measuring materialsand the count materials were examined with X~2test and t-test respectively, P<0.05difference was statistically meaningful.Results:1、The clinical curative effect and indicators1.1The comparison of clinical curative effect between before and after drug use:As for the treatment group of APC,5cases were markedly effective,18cases wereeffective,7cases had no effect. The total effective rate was76.7%. As for the controlgroup of APC, the corresponding result was2,7,21respectively. The total effective ratewas30.0%. There was significant difference between two groups (P<0.05). As for thetreatment group of VPB,6cases were markedly effective,16cases were effective,8had no effect. The total effective rate was73.3%. Accordingly, the result of the controlgroup of VPB was1,9,20respectively. The total effective rate was33.3%. There wassignificant difference between the two groups (P<0.05).1.2The comparison of QTcd in treatment group between before and afterreceiving medication: The treatment group of APB, QTcd from (60.21±18.71)msto(51.03±16.20)ms, P<0.05. The treatment group of VPB, QTcd from (61.98±17.05)ms to(52.23±19.02)ms, P<0.05.1.3The comparison of time domain index of HRV in treatment group before andafter drug use: The treatment group of APB, SDNN from (90.9±24.8)ms to(117.9±25.4)ms, P<0.05. SADNN from (87.8±25.3)ms to (111.0±26.2)ms, P<0.05.RMSSD from(27.0±13.1)ms to(29.6±14.7)ms, P<0.05. PNN50from(7.8±4.2)%toPNN50(9.3±5.5)%, P<0.05. The treatment group of VPB, SDNN from (91.5±24.7)msto (120.6±25.9)ms, P<0.05. SADNN from(89.2±25.1)ms to(113.9±26.5)ms, P<0.05.RMSSD from(26.3±13.7)ms to(28.8±14.6)ms, P<0.05. PNN50from(7.9±4.4)%toPNN50(9.7±6.1)%, P<0.05.1.4The comparison of the24-hours average heart rate in treatment group beforeand after drug use: The treatment group of APB, the average heart rate from (73±10)bpm to (72±9)bpm, P>0.05.The treatment group of VPB, the average heart rate from(75±12) bpm to (74±11)bpm, P>0.05.2. Safety analysis: The blood, urine, stool routine, liver and kidney function, electrolytes were done in60cases patients of the treatment group and showed noabnormalities before and after drug use, which suggested the drug has no obviousadverse reactions and toxic and side effects.Conclusion: The results showed that:1Wenxin granule had significant curativeeffect in treatment of frequent ventricular premature beat and reducing premature beatfrequency, and did not reduce the average heart rate.2It can reduce the QTcd of APBpatients and VPB patients.3It can improve HRV and autonomic imbalance.4Wenxingranule had exact curative effect in treatment of frequent premature beat. It can alleviateclinical symptoms of arrhythmia caused by different kinds of basic diseases safely andreliably, which showed no obvious adverse reaction, taking convenient, bettercompliance, and had no effect on arrhythmia. Wenxin granule is worth to be widelyapplied in clinical.
