BackgroundBone marrow edema syndrome (BMES)of femoral head has characteristic of acute pain, activity limitation and self-limited. There is obviousdifference in pathology and prognosis between BMES and avascularnecrosis of femoral head. At present, the disease been treated by conservative methods mainly, including non-steroidal anti-inflammatory drug,hyperbaric oxygen therapy and physical therapy. The results has keepunclear. Pursuiting a more effective method of treatment is the focus.ObjectiveStudy the effect of core decompression in the treatment of hip bonemarrow edema syndrome (BMES)Methods26patients suffer from femoral bone marrow edema syndrome hadbeen treated during2010January to2012January in our hospital. They weredivided into two groups randomly.12cases of non-steroidalanti-inflammatory analgesic drugs expectant treatment,14patientsunderwent core decompression. The two groups of patients were evaluatedby Harris score and the WOMAC Osteoarthritis Index Scores at pre and posttreatment respectively. During1-2years follow-up, resume normal life time,analgesic drug use time, and duration of pain relief were recorded.Application SPSS16.0statistical software for statistical analysis.ResultsThere were no significant difference in Harris score before treatmentcompare conservative treatment group with core decompression groups (P =0.226). In post-treatment, no significant statistical difference of Harrisscore between conservative treatment with core decompression treatmentgroups (P=0.183). Before treatment, the total score WOMAC inconservative treatment group was7.14±0.69, core decompression groupwas6.77±0.52. No statistically significant between the two groups(P=0.127). After treatment, the total score WOMAC in conservativetreatment group was1.40±1.63, compared with1.18±1.46in coredecompression group, no statistically significant differences between thetwo groups (P=0.609). As pain relief time, the conservative treatment groupwas significantly longer than core decompression group (P <0.001). Coredecompression group resumed daily life time significantly earlier than theconservative treatment group (P=0.002), but no significant difference tosports activity time (P=0.670). The conservative treatment group andmedullary core decompression treatment group has a case of avascularnecrosis of femoral head respectively, incidence rate showed no significantdifference (P=0.720). The two groups had no treatment relatedcomplications.ConclusionsThe clinical effects of two treatment methods finally reached the same,but the core decompression could accelerate the pain relives, shorten theperiod of restore normal life compared with conservative treatment...
