Objective To systematically review and evaluate the safety andefficacy of ultrasonic and monopolar electrosurgical scalpel in laparoscopiccholecystectomy.Methods All randomized clinical trials in which ultrasonic ormonopolar electrosurgical scalpel during laparoscopic cholecystectomy withthe adult patients of symptomatic gallstone disease were searched fromPubMed,Cochrane library,EMbase, Ovid,CNKI,WanFang and VIP.Tworeviewers independently assessed study quality and extracted data. All datawere analysed using RevMan5.1.Results Eight high-quality randomized controlled trials with1056patients were included. Differences in mean operation time, mean blood loss,mean hospital stay, gallbladder perforation, and post operative abdominalpain score at24hours were statistically significant between the two groups,in favor of the use of ultrasonic scalpel. However, there were no differencesin operation conversion, bile leakage, intra-abdominal collections, andpostoperative nausea at24hours. Conclusions The results show that compared to monopolarelectrosurgical scalpel, the use of ultrasonic scalpel is safe and effective andpotentially might be safer in LC.
