Objective:To compare the advantages of critically ill neonatal mortalityrisk prediction between NCIS and SNAPⅡ, to find out whether SNAPⅡ isthe better neonatal disease severity scoring system feasible for use in ourcountry.Methods:Cases into groups have be scored while simultaneouslyusing two scoring system,according to the scores the patients were dividedinto three groups(very critical,critical,noncritical group),each groupmortality rate was calculated and the corresponding mortality betweengroups were compared matched-pairs.The discriminatory ability of thescores Was assessed measuring the area under the receiver operatingcharacteristic curve(AUC),the outcome’s measurement was mortalityrate;cases in2011.11and2012.11have be scored by SNAPⅡ,according tothe scores the patients were divided into three groups(very critical,critical,noncritical group),each group mortality rate was calculated and thecorresponding mortality rate between groups were comparedmatched-pairs,is to compare the medical standard between the year2011and2012in our hospital’s NICU.Results:The total mortality rate in thisgroup is13.4%(20/149).The comparison between the three groups of SNAPⅡ score:the differences were statistically significant in mortality rate between the three groups (P<0.00;X2=33.620);and the higher of the SNAP Ⅱ score,the higher risk todeath;The comparison between NCIS first score and SNAPⅡ score:thedifferences were not statistically significant in mortality rate between thethree groups (p>0.05);The comparison about the SNAPⅡ score betweencases in2011.11and cases in2012.12: the differences of SNAPⅡscore andmortality rate between the two groups were not statistically significant(p>0.05);The spearman correlation coefficient between hospitalization time,SNAPⅡ is-0.042, NCIS is0.074;The area under the ROC curve(AUC),NCIS first score0.704,SNAP Ⅱ score0.901, respectively.Conclusion:SNAP Ⅱand NCIS first score are both nice morbiditypredictors for critically ill neonatal, but SNAP Ⅱscore Can predictabandoning risk in newborn earlier and fairly accurate and clinicalapplication is easier by comparison,so SNAPⅡ score should be usedgenerally in our country;SNAPⅡ and NCIS have some correlation withhospitalization time;SNAP Ⅱs core can be used to compare the medicalstandard between NICU and to promote the medical standard of ourcountry to some extent.
