Background Liver failure is a group of severe clinical syndromescaused by a variety of pathogenies.Because of the dysfunction of liver,it iseasy to occur with acid-base imbalance.Metabolic acidosis is a very severereason that disturbs the balance of homeostasis and makes many systemsand organs failure,end with a bad outcomes.Acid production includeslactate,orthophosphoric acid and so on.The accumulation of lactate meansmore catabolism and less metabolism,ischemia and hypoxia,the dysfuctionof liver for thansfering and metabolism.All of them prevent cori cycle fromrecycling.It is a vicious circle if our orgams accumulate more and morelactate and cause more serious metabolism disturbance.Blood lactate levelhas became an objective index to estimate the condition of liver failurepatients.Most of the liver failure are caused by HBV,a large number ofthem take oral nucleoside analoguses to inhibite the replication ofHBV.Because nucleoside analoguses’ pharmacology are inhibite DNApolymerase to stop the virus replication.In theory,it also inhibitemitochonidrial DNA replication,causing cellular damage.To discuss therealationship between blood lactate result with the other related index which were observed during the hospitalization with liver failure patients.Methods Selected clinical data of liver failure patients hospitalized inInfectious Department of the Second Affiliated Hospital of ChongqingMedical University from January2011to October2011,then analysis theirrelationship between LC with these data.Results There were no statistically significant about the relationshipbetween LC with sex、age、the pathogenies of liver failure and whether withascites or not,but have significant in the analysis the relationship betweenLC with the outcomes、infection、PTA、hepatic encephalopathy and hepaticrenal syndrome.The p value of outcomes,PTA and hepatic encephalopathyare0.000.Infection’s p value is0.023,and hepatorenal syndrome is0.043.Conclusions It’s the condition of liver failure that cause the increaselactate level,also caused by infection、 hepatic encephalopathy andhepatorenal syndrome,but have no significant relations with taking oralnucleoside analoguses.Discussion Because the lactate level is relative with the conditionitself,maybe control the condition in early stage can maintain lactate levelon stable.Nowadays,it has became a top topic about the treatment withglucocorticoids for liver failure.It could prevent part of the liver cells fromnecrosis and reduce the mortality.However,the side effects ofglucocorticoids become the obstacle whether use this to treat or not.In2006,Chinese 《Guidelines for the diagnosis and treatment of liver failure》 recommended that without infections,bleeding or any other manifestationsglucocorticoids could be used for liver failure in early and medium stage.
