Objective: To analyze the short-term efficacy of simple discectomy for different degreesof disc degeneration in LDH in order to propose a treatment guideline.Methods: Retrospective analysis was carried out in85patients who underwent simplediscectomy for lumber disc herniated (LDH) from June,2012to March,2013in ourhospital. The age, gender,preperative and postoperative score of VAS and ODI index wererecorded and the data of imaging was analyzed. Firstly, according to age (year),the patientswere divided into3groups:<30group,30-50group,>50group;Secondly, according to theendplate Modic type, Woodend type and the type of lumbar disc herniation (the includedand the not included), preperative and postoperative score of VAS and ODI index werecompared. Statistical Methods was used to analyze the efficacy.Results: There were41patients completed more than1year follow-up study bytelephone, including27men and14women, which average age was37.8and44.4yearsold.1.The difference of preperative and postoperative score of VAS and ODI indexbetween all the patients was statistically significant, P<0.05.2.Compared with <30groupand the other two groups, the <30years old group of postoperative score of VAS and ODIindex was significantly higher than the other two age groups, P <0.05; the postoperativescore of VAS and ODI index of other age groups was no statistically significant difference.3. Among the different endplate Modic types, the difference of preperative score of VASbetween endplate Modic type1and endplate Modic type0was statistically significant,P<0.05. Postoperative VAS score and ODI index between the all groups, there was nostatistically significant difference.4. Among all Woodend type:the preoperative andpostoperative VAS score, ODI index of intervertebral disc of Woodend Ⅱ, Ⅲmodel groupwas significantly higher than disc Woodend Ⅳmodel group, P <0.05; the preoperative andpostoperative VAS score, ODI index of intervertebral disc Woodend Ⅱ type and Ⅲtype between was no statistically significant difference; Postoperative VAS score of WoodendⅣ group was obviously higher than that of intervertebral disc Woodend Ⅱ, Ⅲtype group,P <0.05, there was no statistical significance in ODI index of the three groups.5.Compared with the different types of LDH, Preoperative VAS score, ODI index of thecontained of lumbar disc herniation was significantly higher than the not contained group,P <0.05. At the time of the last follow-up after the VAS score and ODI index between thetwo groups, there was no statistically significant difference ODI index.Conclusion:1.There are some factors are limited influence of lumbocrural pain andfunction in patients with LDH in preoperative: endplate Modic1degeneration,intervertebral disc Woodend Ⅱ, Ⅲ change andthe included of lumbar disc herniation,butthe endplate Modic degeneration and herniated type does not affect the curative effect ofsimple discectomy.2. The effect of simple discectomy curative in the LDH patients is notobvious,because of patients after age <30years old, intervertebral disc Woodend Ⅳtypechange, so we should choose other way to improve the patient’s quality of life.
