Objective To observe the effect of glucocorticoid in patients with heavy tendency hepatitis or early-stage liver failure which induced only by HBV, and to investigate the blocking role and safety of glucocorticoid in the course of hepatitis B to deteriorate.Methods60patients with heavy tendency hepatitis or early-stage liver failure, divided into glucocorticoid treatment group and the control group.The control group was used the conventional methods which included protecting liver, redocing jaundice and antiviral treatment, while the glucocorticoid treatment group on the basis of conventional therapy combined with short-term methylprednisolone therapy. The liver function, prothrombin activity (PTA), severe hepatitis incidence rate,side efect incidence rate and mortality of the two groups were compared after therapy for four weeks. Results After four weeks therapy, the blocking rate of severe hepatitis in glucocorticoid treatment group was82.76%, higher than that in control group, it had statistical diffbrence(P<0.05). Compare to the control group, total bilirubin (TBil) decreased and PTA increased significantly in glucocorticoid treatment group patients after therapy for four weeks. There were statistically significant differences between the two groups (P<0.05), but there were no significant statistical differences between the two groups in side effect incidence rate(P>0.05). and the severe hepatitis incidence were lower than the control group with statistical difference (P<0.05).Conclusion The patients with heavy tendency hepatitis or early-stage liver failure which were given short-term methylprednisolone treatment in the early stage maybe can reduce the incidence of severe hepatitis.
