Objective:To investigate the possible cause of postoperative liver metastasis of colon cancerpatients,predict the risk factors of postoperative liver metastasis of colon cancer patients,provide advice to early diagnosis of postoperative liver metastasis of colon cancer patients.Methods:306colon cancer patients who has received radical surgical treatment collected inour hospital from2004to2009year. Including72cases of colon cancer patients with livermetastasis,234cases without liver metastasis.choose the gender, age, blood type, tumorlocation, tumor long diameter, invasion degree, degree of differentiation, tissue type, generaltype, vascular invasion, lymph node metastasis, number of lymph node dissection,preoperative CEA levels, preoperative anemia, blood transfusion status, operation time andwhether chemotherapy, a total of18clinical pathologic factors to single factor andmulti-factor analysis.Results:There were72patients with liver metastasis in this group, the incidence was23.5%,there were41males and31females, the male-to-female ratio was1.32:1.In this group,21cases less than50years old and51cases more than50years old, with a median age of58.5years.16cases of blood type A,19cases of blood type B,type AB blood of17cases,20casesof type O blood.30cases of Sigmoid,12cases of descending colon,13cases of transversecolon,7cases of ascending colon,10cases of ileocolonic.Tumor diameter less than or equalto5cm34cases,6-8cm28cases,greater than equal10cm10cases. Submucosa in1case,8cases of muscle layer,subserosal37cases, intestinal wall26cases,46patients were middledifferentiation and3patients were low differentiation.Two cases of papillaryadenocarcinoma,57cases of tubular adenocarcinoma,10cases of mucinous adenocarcinoma,3cases of signet ring cell carcinoma.10cases of the elevated type,57cases of ulcerscarcinoma,five cases of infiltrating carcinoma.10cases of non-infringement Vasculature,62cases of infringement Vasculature.Four cases of lymph node metastasis,lymph nodemetastasis68cases.Lymph node dissection is less than or equal to12was53cases,lymphnode dissection is greater than12was19cases.Preoperative CEA level less than or equal to5mmol/L was23cases, greater than5mmol/L was10cases,39cases was notchecked.Preoperative no anemia was41cases, Preoperative no anemia was31casesPreoperative obstruction was58cases, preoperative obstruction in was cases.No bloodtransfusion was37cases,blood transfusion was35cases.surgical time less than3hours were20cases,surgical time more than3hours were52cases.postoperative chemotherapy was onecases,Univariate analysis showed that patient age, tumor differentiation, gross type, vascularinvasion, lymph node metastasis, number of lymph node dissection, preoperative CEA level, operative time affect liver metastases (χ~2=5.912,50.474,9.451,118.019,83.631,7.137,8.332,12.691,P<0.05)。The logistic regression analysis showedthat the degree of tumor differentiation, vascular invasion and lymph node metastasis wereindependent risk factors of liver metastasis.(b=1.445,2.504,2.469,P<0.05)。Conclusions:1.Patient age, tumor differentiation, gross type,vascular invasion,lymph nodemetastasis,number of lymph node dissection,preoperative CEA levels,duration of surgeryaffect the colon cancer patients with liver metastasis.2.Poor tumor differentiation,vascular invasion and lymph node metastasis increased risk ofcolon cancer patients with liver metastasis.
