Objective: To improve the autism behavior scale mainly used in community onearly autistic spectrum disorders. And explore its reliability and validity. Alsoinvestigate how the screening network program and management processes work, sothat to provide favorable conditions for the early intervention of children with AutismSpectrum Disorders (ASDs).Methods: Based on the clinical manifestation and diagnostic standard of autismin DSM-IV, referred by CBS,CHAT,CARS,ADI-R,we improved the autism behaviorscale.Randomly select6~24month infants as a screening object, test reliability andvalidity of the scale. The improved scale applied to the community early screening ofASDs.Establish community screening network and management process.Three levelsof screening and diagnosis management network were established, include based level, second level and third level. The community health centre as the based level mainlyfor screening, while the hospital of child and women ‘s health care of local area andthe city area as the second and third level for diagnosis and intervention.Results:(1)13000infants were screening by the scale and12141wereeffectively return back. The rate of return back is93.39%;(2)The good inter–raterreliability respectively were0.81~0.96(P<0.01).The good inter–rater of each factorand total score were0.945,0.973,0.874,0.952,0.962.(3)The test-retest reliabilityrespectively were0.85~0.95(P<0.01). Test-retest reliability of each factor and totalscore were0.936,0.910,0.925,0.893,0.919;(4)Split half reliability was0.917;(5)Coefficient of Cronbach’s alpha is0.934.The scale has good internalconsistency;(6)Criterion-related validity was0.958;(7) The sensitivity and specificitywere1.000and0.911, when the boundary value was7points;(8)The based levelnetwork preliminary screening rate of ASDs increased from43.40%to81.03%by theyears, and also, the referral rate increased from38.01%to67.45%;(9)Districtmaternal and child health care accepts positive preliminary screening ASDs children 1129cases, among704cases of normal children,30cases of psychomotor retardationchildren,109cases of language retardation children,286cases of suspected ASDschildren. The positive rate of it was37.64%.Conclusion: The Early Autistic spectrum disorders Screening Scale forCommunity has good reliability and validity, and fit for the screening of early autisticspectrum disorders in community. The establishment of community screeningnetwork and management process increase the screening and referral rates in thecommunity health centre. And it is helpful for the early detection and intervention tothe children with ASDs.
