Background Hemodialysis is one of the important ways to treat the end-stage renaldisease.A good vascular access is key to the smooth progress of the hemodialysis forend-stage renal disease patients.The native fistula, artficial vascule and centre veinduct are the most common vascular access for MHD patients.And the native fistula isthe best maintenance of vascular access because of its longest life and least morbidity.The service life of AVF and its function is closely related to the outcome ofdialysis.According to some result,there were20%MHD patients hospitalized for theirvascular access problem. Arteriovenous fistula thrombosing is one of the most commonmorbidity in maintenance hemodialysis (MHD) patients, and often lead to itsdysfunction. Expliciting the influential factor of thrombosis in AVF will be conduciveto the AVF protection.Although,we can found not a few research about the AVFdysfunction,most of them compared the data between the case group and the controlgroup,without regarding the use period of their AVF.Obviously,the use period isclosely related to the function of AVF.So matching the use period of the AVF betweenthe case and control groups will reduse the influence of this confounding factor,andthe result will be more credible.The following research was desined according to it.Objective To investigate the influential factors of thrombosis in native arteriovenousfistula (AVF) in maintenance hemodialysis patients.Methods46patients were recruited as case group when their AVF were thrombed atthe first time.92patients whose AVF were conducted at the same time with the ratio2:1of each matched case were also recruited as control group. The clinical andbiochemical data of the two groups were analyzed by analysis of variance, binarylogistic regression analysis and survival analysis.Result Diabetes, hemodialysis frequency, hypotension, triglyceride,calcium-phosporous product, cardio-cerebrovascular disease and carnitine werestatistically significant difference between the two groups. By logistic regression analysis, diabetes and calcium-phosporous product were the risk factors of AVFthrombosis, but carnitine was the protective factor.Conclusion Diabetes and calcium-phosporous product could be the risk factors ofAVF thrombosis, and carnitine injected may be a protective factor.
