[Purpose] Nowadays, The cause of intrahepatic bile duct stones is reviewed as that the formation of biliary stone subsequent changes the hepatic inflow, and speeds up the stone’s fomation in many reserches.While some studies found that more and more intrahepatic biliary cholesterol stones regardless of biliary infection or obstruction. This phenomenon reminds us that intrahepatic bile duct stone’s formation not only exist in the changing of the biliary tract micro environment, but also in transformations of hepatocytes or bile duct cell metabolic functions. Infection、 obstruction and some other factors may be the motivators, rather than real initiating agent. The initial factors probably lies in the altered liver cell function, while liver cell function is the inevitable result nutrition metabolic changes in hepatic cells. we bileve that the changes in hepatic blood flow result in stone before the hepatocyte funtion damaged.In Clinical researches, we studied that the first hepatic portal vein near the door of hepatic artery’s average velocity contrast flow and normal speed in patients with intrahepatic stone,to explore the relationship between biliary fluid mechanics and calculus of intrahepatic bile duct stones.[Methods] Collecting patients in my hospital who suffered form intrahepatic bile duct stones form January in2011to March in2013.Using color Doppler to detect the first hepatic portal vein near the door of hepatic artery mean velocity value, compared with35healthy volunteers without hepatobiliary diseases and others.In the end, analysing the data with SPSS16.0.[Basis]1Intrahepatic bile duct only be filled with hepatic artery.2The first hepatic portal vein near the door of hepatic artery in Colour Doppler Ultrasound detection is easy to show, That is because of not only its anatomical fixed position,but also we can make the main portal vein as a reference, the two section of hepatic artery of normal display rate of up to100%.3Compared with the portal vein,the hepatic artery, have more muscular, easily affected by sympathetic nerve.[Results]1Comparing the factors of primary intrahepatic bile duct stone patients in working condition and moods,we found the primary intrahepatic bile duct stone patients swing mood usually, it may affect the formation of stones.2Colour Doppler Ultrasound detection results of primary intrahepatic stone patients’artery flow velocity on average faster than healthy volunteers, there was no significant difference of the portal vein flow.3Through the SPSS16.0in patients with primary intrahepatic bile duct stone processing of experimental data, the average velocity of hepatic artery had statistically significant difference compared with healthy people, no statistically significant difference between the portal venous velocity.[Conclusion] The formation of the primary intrahepatic bile duct stone may be related to changes in biliary tract rheology and blood rheology, while the sphincter of Oddi function degradation and gastrointestinal bacterial translocation may cause the formation of extrahepatic bile duct stones.That is to say,the formation of extrahepatic biliary calculi promotes the intrahepatic bile duct stones, both of them resulted by biliary tract micro environmental changing and chronic proliferative cholangitis.While the abnormal stress state cause the support and oxygen reducing, this may promote the primary factor of this formation.
