Purpose Explore the pathogenesis of single lacunar infarction in patients within deep perforating artery territoryMethod A total of113patients single lacunar infarction in the cerebral deep perforating artery territory within48hours of stork onset admitted between October2011to November2012at the First Affiliated Hospital of Kunming Medical University,Department of Neurology.1.We divided the lacunar into2groups according to the presence of parent disease:(1) Single lacunar with parent artery disease;(2) Single lacunar without parent artery disease. The prevalence of cerebral small vessel disease indicators (Leukoaraiosis and enlarged perivascular space) and atherosclerosis indictors (cerebral athersclerosis,dyslipidemia,diabetes and coronary heart disease) were compared among the two groups.2.we also divided the single lacunar into4groups according to lesion location since the difference of parent artery:(1) The blood supply to the perforating artery territory of middle cerebral artery;(2) The blood supply to the perforating artery territory of internal carotid;(3) The blood supply to the perforating artery territory of posterior cerebral artery;(4) The blood supply to the perforating artery territory of vertebral artery. The prevalence of SVD indicators, atherosclerosis indictors and arterial lesions number were compared among the four groups.Results1.Single lacunar with parent artery disease had the highest prevalence of atherosclerosis indicators and lowest prevalence of SVD indicators,whereas single lacunar without parent artery disease had the lowest prevalence of atherosclerosis indicators and highest prevalence of SVD indicators. Atherosclerosis indicators significantly increased and SVD indicators significantly decreased as the vasular territory lower(from the middle cerebral artery, the internal carotid,the posterior cerebral artery,the vertebral artery).Conclusion Differences in SVD and atherosclerosis indicators suggest that Single lacunar has a heterogeneous pathogenesis according to lesion location and the presence of PAD.
