Objective:preliminary study of SD rats with biliary obstruction and recanalization of postoperative peripheral blood bilirubin and multidrug resistance-associated protein2(MRP2) changes of liver tissue expression. Investigate abnormal expression of rat MRP2protein to bilirubin excretion and the relationship between obstructive jaundice organ damage.Methods:90adult female SD rats were randomly divided into three groups, namely the establishment of sham surgery (SH,n=30), obstructive jaundice (OJ,n=30), biliary drainage (ID,n=30) model. First surgical OJ group and ID group to establish obstructive jaundice model, SH group separation of the common bile duct, Reared seven days after,The second surgery ID group establish bile drainage model, SH and OJ group only separation of the common bile duct. At3,5and7days after the first postoperative, Specimens from the SH group and the the OJ group of blood samples and liver tissue specimens, At3,5and7days after the second postoperative, Specimens from the ID group and the the OJ group of blood samples and liver tissue. The detection comparison groups of animal serum total bilirubin (TB), albumin (ALB), alanine aminotransferase (ALT), aspartate aminotransferase (AST) level and pathological changes of the liver tissue. Groups of MRP2protein expression levels in liver tissue was measured using reverse transcription-polymerase chain reaction (RT-PCR) method, And SABC immunohistochemical methods determine MRP2protein expression in normal liver tissues, HE staining observed pathological changes of the liver tissue.Results:the successful establishment of the three groups of rats animal models.3,5,7days after bile duct ligation, The OJ serum bilirubin concentration (176.62±8.65,227.00±8.36,225.38±5.56),3,5,7days after the second operation.The ID serum bilirubin (55.16±5.01,20.58±6.16,5.34±0.48) continued to decline, OJ group (234.74±10.14,236.22±11.08,236.34±10.63) serum bilirubin continued to maintain a high level. ALT, AST, and other the enzymatic indicators also showed the same changes. SH group liver pathology examination showed normal lobular structure, Bile duct ligation, the portal area of liver, lobules surrounding visible newborn bile duct-like epithelial cells, inflammatory cell infiltration, original intrahepatic bile duct epithelial cells of the small amount of shedding and hepatocyte necrosis.OJ group after second surgery for7days, Inflammatory cell infiltration of the portal area, bile duct hyperplasia, the large liver necrosis bleeding lesions, tissue structure.ID group biliary-enteric anastomosis the visible liver cell hyperplasia, lobular architecture is gradually returning to normal, no obvious inflammatory cell infiltration in the portal area, still visible a small amount of fibrous tissue hyperplasia. Common bile duct ligation on day3,5,7, MRP2protein expression levels in rat liver was significantly lower than the SH group, was a persistent reduction (OJ VS SH, P<0.05), After the second surgery, The the OJ group MRP2protein expressed at low levels significantly decreased with time; Biliary-enteric anastomosis, The ID group after MRP2protein expression gradually increased and gradually returned to normal levels (ID VS SH, P0.05) with time.Conclusion:the rat biliary obstruction early liver obvious damage, The body can produce certain compensatory to improve liver function, However, the persistence of the biliary obstruction ultimately lead to liver fibrosis, liver failure, and even lead to the occurrence of MODS. MRP2protein expression levels change with obstructive jaundice and biliary recanalization level of bilirubin levels have a significant correlation. MRP2protein in rats with obstructive jaundice to reduce the level of transcription and protein expression, levels of bilirubin expression, Hyperbilirubinemia liver cells to bilirubin excretion barriers, lead to further aggravate the body, thereby increasing the liver function and organ damage. And biliary-enteric anastomosis, The MRP2protein transcription and protein expression levels increased, levels of bilirubin expression, both statistically significant negative correlation.MRP2protein is of great significance in the protective effect on the excretion of bilirubin in the bile and liver.
