[abstract] objective:To discuss the predictive value of cervical disease of P16INK4A protein, and the relationship between high-risk type HPV infection in HIV-infected women in yunnan province, hoping to find a convenient and cheap screening program for HIV-infected women. Methods:HPV detection, and diagnostic colposcopy were followed by cervical biopsy if indicated. We took67cases as the research object who were performed the cervix biopsy from301HIV-infected women voluntary for cervical cancer screening.13out of67without cervical intraepithelial neoplasia and cervical invasive cancer as control group according to cervix biopsy. Logistic regression analysis was used to analyze associations between high-risk human papilloma virus (HPV) infection and expression of P16INK4A protein, and analysed the the predictive value of P16INK4A protein. Results:The correlation exsisted between the expression of P16INK4A protein and high-risk HPV infection (r=0.342; P=0.025<0.05), but not multiple high-risk HPV infection (P>0.05); the difference of P16INK4A protein expression rate btween low grade cervical intraepithelial neoplasia (CIN1group) and high grade cervical intraepithelial neoplasia (CIN2+CIN3group) was statistically significant (X2=20.053, P<0.05), shows that P16INK4A protein can distinguish low grade cervical intraepithelial neoplasia from high grade cervical intraepithelial neoplasia; as the grade of cervical intraepithelial neoplasia increasing, the expression of P16INK4A protein increased strength too, linear correlation.(R=0.397, P<0.05).Conclusion:P161NK4A protein immunohistochemical detection is closely associated with high risk type HPV, have predictive value for cervical intraepithelial neoplasia, is able to distinguish low grade cervical intraepithelial neoplasia from high grade cervical intraepithelial neoplasia, with some predicted value in the progress and evelopment direction of cervical intraepithelial neoplasia, can be considered as a way of cervical cancer screening for HIV-infected patients in the future.
