Objective To detect the exPression of inflammation factors (TLR2、TLR4and TNF-a) in Placenta tissue of obese Pregnant women, evaluate and analyse the correlation between obesity and Placental inflammatory factors. To investigate the effect of weight control during Pre-Pregnancy and Pregnancy.Materials and methods Collected gravidas from January2011to May2012in central Tianjin HosPital of Obstetrics and Gynecology, with no comPlications of Pregnancy, delivery of a full-term Pregnancy, divided into4groups according to Pre-Pregnancy BMI and weight gain during Pregnancy. Including group of normal weight in Pre-Pregnancy and normal weight gain during Pregnancy(group Ⅰ:normal group, n=35), group of normal weight in Pre-Pregnancy and overweight growth during Pregnancy (group Ⅱ,n=31), group of obesity in Pre-Pregnancy and normal weight gain during Pregnancy(group Ⅲ, n=22), group of obesity in Pre-Pregnancy and overweight growth during Pregnancy (group Ⅳ,n=38), resPectively. Detecting the exPression of TLR2, TLR4and TNF-a in Placenta all groupswith immunohistochemical assay. Detecting the level of blood liPid. coagulation Parameters of4groups of Pregnant women, and measuring weight of the neonatal birth.Result(1) The exPression of TLR2in membrane and/or cytoPlasm. The Positive exPression rate of Placenta tissue TLR2in group Ⅰ, Ⅱ, Ⅲ,Ⅳ were34.29%、70.97%、72.73%、92.11%, resPectively. group Ⅱ、Ⅲ and Ⅳ were significantly higher than group Ⅰ (P<0.01).group IV were significantly higher than group Ⅱ、 Ⅲ (P<0.01).There was no significan difference between group Ⅱ and group Ⅲ(P>0.05)(2) The exPression of TLR4in membrane and/or cytoPlasm. The Positive exPression rate of Placenta tissue TLR4in group Ⅰ, Ⅱ, Ⅲ,Ⅳ were28.57%.67.74%、63.64%、94.74%, resPectively、group Ⅱ、Ⅲ and Ⅳ were significantly higher than group Ⅰ (P<0.01).group Ⅳ were significantly higher than group Ⅱ、 Ⅲ (P<0.01).There was no significan difference between group Ⅱ and group Ⅲ(P>0.05).(3) The exPression of TNF-a in nucleus. The Positive rate of exPression of TNF-A in Placenta in group Ⅰ, Ⅱ, Ⅲ, Ⅳ is17.14%、45.16%、90.91%、100.00%, resPectively. The highest exPression was group Ⅳ, the second was group Ⅲ, the third was group Ⅱ, the lowest was group Ⅰ (P<0.01).(4) In Placental tissues of Pregnant women of all groups, both the exPression of TLR2and TLR4have Positive correlation with the exPression of TNF-a(r=0.335, P<0.01; r=0.231, P<0.05); the exPression of TRL2and TRL4was Positively correlated(r=0.329, P<0.01)(5) ComPare of biochemical indicators for blood liPids in4groups of Pregnant women:group of excessive weight gain (group Ⅱ,Ⅳ) total cholesterol and aPoA were higher than normal weight gain group(group Ⅰ,Ⅲ)(P<0.05); In contrast HDL, group Ⅰ、Ⅲ were higher than group Ⅱ、Ⅳ(P<0.05).There were no different of TG、 LDL、 aPoB between4groups(P>0.05).(6) ComParison of coagulation Parameters of Pregnant in four groups:fibrinogen and D-dime of Pregnant in group Ⅱ, Ⅲ and Ⅳ were higher than group Ⅰ (P<0.01). There was no significant difference between group Ⅱ, Ⅲ,Ⅳ(P>0.05)(7) The weight of neonatal is influenced by the weight gained during Pregnancy. The group of excessive weight increasing during Pregnancy(Ⅱ,Ⅳ)were higher than group Ⅰ (P<0.01). There was no statistically significant difference between the other groups (P>0.05).Conclusion (1) The abnormal exPression of TLR2、TLR4、TNF-a in Placenta tissue of obese Pregnant women. Obesity may induce inflammation in Placenta tissue,Pre-Pregnancy obesity and excess weight gain during Pregnancy(obesity during Pregnancy) are associated with inflammation in Placenta tissue.(2)ExPression of three inflammatory factor TLR2、TLR4、TNF-a has relevance in Placental tissue of obese Pregnant woman, and it is Positive correlation.(3) Obesity can cause blood liPids of Pregnant women increased, leading to hyPercoagulable state and neonatal birth weight increased, of which Pregnancy weight growth in excessive (obesity during Pregnancy) as the main influencing factors.(4) So we need to strengthencontrol of weight in women before and in gestation.
