Objective:To investigate the characters of images and treatment for surgicaltreatment for temporal lobe AVM,and discuss the curative effect of the surgical approach.Methods: The images date and clinical data of16patients with temporal lobe AVMs, whounder went microsurgery from October,2010to October,2012,were analyzedretrospectively.13patients were received microsurgical exclusion and3patientsendovascular embolization. The amount of perioperative blood, postoperative Glasgowoutcome scale (GOS) and complications were observed and compared. Results: DSAshowed that the typical AVMs were a characteristic honeycomb vascular mass with bigfeeding arteries and draining veins. The diameters of AVMs were less than3cm in6cases,and ranged from3to6cm in10cases.8patients presented with brain hemorrhage,3patients with epilepsy and4patients with headache. Total focus removal was achieved in10cases,3cases with not completely were received postoperative management aboutĪ³-ray.According to GOS score,11patents got good recovery,2moderately disabled,3severely disabled,0plant survival and0died.Range60%embolization in2cases,under50%in1case.Conclusion: The diagnosis value of evaluating in medical imagingincluding CT,MRI,CTA,MRA and DSA on angioarchitecture imaging and its relation tocircumstrueture of AVM has advantage and disadvaniage itself,which has a guidance forseleeting a treatment role. The microsurgical treatment for temporal lobe AVMs has beengreatly restrained by the involvement of multiple brain functional regions and its anatomiccomplexity.A more aggressive oproach is recommended.Microsurgical exclusion showsobvious advantages in less amount of perioperative blood, better efficacy and high safetyin the treatment of temporal lobe arteriovenous malformation, thus it is worthy of appliedin clinic.
