Objective: To investigate the effect of benazepril to the serum level of apelin-12inEH patients and EH patients with microalbuminuria and its clinical significance.Methods:40patients(male:23, female:17) of mild to moderate hypertension patients(Chinese Hypertension Prevention Guide for2010standard) and40patients (male:19,female:21) of hypertension with urine microalbumin were selected from University ofSouth China Second Affiliated Hospital Cardiology Department and30controls(male:14, female:16) with normal blood pressure were selected from University ofSouth China Second Affiliated Hospital Medical Examination Center from March2011-March2012.4ml venous blood were collected from each selected candidatesfor apelin-12concentration testing by ELISA. Meanwhile, color ultrasound was usedto measure the carotid intima-media thickness of all. EH group and EH with urinemicroalbumin group were treated with benazepril for26weeks, and the control groupwere given placebo for26weeks. After treatment,4ml venous blood were collectedfrom each selected candidates for apelin-12concentration testing by ELISA.Meanwhile,24hour urine protein and the carotid intima-media thickness were alsomeasured.Results:1.There was no significant difference of general biochemical parameters(hepatorenal function, blood lipids, blood sugar), body mass index (BMI) and leftventricular ejection fraction, left ventricular end-diastolic diameter (LVEF, LVED)among each group(p>0.05); Hypertension group and hypertension withmicroalbuminuria group showed a robust increase of blood pressure compared withthe control group (P <0.05), but there was no significant difference of blood pressurebetween hypertension group and hypertension with microalbuminuria group (P>0.05).2. The hypertension group displayed a dramatic decrease of serum apelin-12levelcompared with the normal control group(P<0.05).The hypertension with microalbuminuria group displayed a drastic decrease of serum apelin-12levelcompared with the control group(P <0.01). The hypertension with microalbuminuriagroup displayed a prominent decrease of serum apelin-12level compared with thehypertension group(P <0.05). The hypertension with microalbuminuria group showeda significant increase of C-IMT compared with the other two groups(P <0.05). Thehypertension group showed a mild increase of C-IMT compared with the controlgroup (P>0.05).3. After treatment of benazepril for26weeks, the hypertension group and thehypertension with microalbuminuria group demonstrated a dramatic increase of serumapelin-12level(P <0.05). Moreover, after treatment of benazepril for26weeks, thehypertension with microalbuminuria group demonstrated a drastic decrease of24houralbuminuria and carotid intima-media thickness (P <0.05).Conclusion:1. Apelin-12has a guiding significance to early As.2、Benazepril could reduce the blood pressure probably by elevating the level ofapelin-12and decrease the concentration of microalbuminuria probably by Inhibitingvascular remodeling.
