ObjectivesTo explore the application of the combination of coventional smear and liquid-basedpreparations technique in improving positive detective rate of thyroid fine-needleaspiration cytology.1. To arrive at the strengths and weaknesses which compare coventional smear andliquid-based preparations(LBP) technique in the cytologic diagnosis of thyroidneedle.2. Under the verification of histopathological results,to synthetically evaluate andanalyze of the diagnostic result of the coventional smear(CS),LBP and thecombination,and to further explore specific method to improve the diagnosticaccuracy and positive detective rate of thyroid needle by utilising both advantages.MethodsFrom August2009to August2012,204thyroid needle patients were get fine–needleaspiration cytology that based on histopathology and from our hospital outpatient,hospitalized, consultation patients. All specimens were divided into half, which wereprepared with LBP and CS technique respectively and observed, analyzed, diagnosedby microscope. Comparative analyze the two specimens quality and its morphologicalfeatures. comparative respectively LBP, CS, combined diagnostic results andhistopathological results, calculate the sensitivity,specificity, accuracy, predictive valueof positive test and predictive value of negative test. Systematically comparative andanalyse the two cytology and the combined diagnostic results. Results1. Specimen quality and morphological characteristic: Compared to CS, LBP havemoreeffective diagnostic cells and clean background, its follicular epithelial cellsmorphology and arrangement is clearer, nucleolus is more typical, cells overlapsare significantly reduced, phenomenon of evenly monolayer arrangement is morecommon. however, in contrast to CS, effective auxiliary ingredients of LBP suchas colloid, inflammatory cells, follicular epithelial tissue structure, etc retained isfewer, which affect differential diagnosis of some thyroid diseases.2. Evaluation indicators of LBP, CS and combination diagnostic: the sensitivity ofLBP(65.9%) is higher than CS(54.5%), its specificity(92.5%) is slightly lowerthan CS(95.6%), both differences have no statistical signification, however, thesensitivity of consociation(75.0%) is higher than CS significantly, itsspecificity(88.1%) is lower than CS, both differences are statistical significance;the accuracy of LBP and CS are both86.8%, the predictive value of positive test ofLBP(87.9%) is lower than CS(92.3%), the predictive value of negative test ofLBP(92.5%) is higher than CS(89.0%), both differences have no statisticalsignification; both the accuracy(85.3%) and the predictive value of positivetest(84.6%) of consociation are lower than CS, its predictive value of negative testis higher than CS, both differences have no statistical signification.Conclusions1. In conclusion, in the slide-preparing technique of thyroid FNAC, LBPsignificantly improved specimen quality and the diagnostic sensitivity of thyroidcancer, LBP slide avoid the disadvantages of CS, it could make background clean,make follicular epithelial cells as well as arranged arrangement typical, also, itcould collect more specimens, but its own limitations influence the accuratediagnosis of some thyroid diseases.2. Combined diagnosis of LBP and CS could improve the positive detective rate of thyroid FNAC compared to either of them. while compare to LBP as well ascombined diagnosis, CS play a important role in differentiating the benign thyroiddisease.
