Objective:Gastric cancer is a common digestive malignant tumor in our country, due to lacks of specific performance, the early gastric cancer is difficult to detect, the majority of patients were find to be advanced gastric cancer, with poor prognosis. The therapeutic modalities on gastric cancer is still surgical predominantly comprehensive treatment. Studies have shown that, Locally advanced gastric cancer using perioperative comprehensive model that the neoadjuvant chemotherapy-operation-and postoperative chemotherapy and/or add local radiotherapy can improve the prognosis. However, how to determine accurate preoperative staging in patients with locally advanced are needed. But using CT or abdominal ultrasound is unable to determine the scope of the gastric lesion invasion, Only by endoscopic ultrasonography (EUS) can solve this problem. In another aspect, postoperative adjuvant chemotherapy has become an important part of comprehensive treatment in gastric cancer.but, there are some problems on how to carry it. Such as using a single joint solution gives6treatments or sequential joint scheme treatment remains unclear. so we make a study that we through preoperative EUS examination results compared with postoperative pathologic staging, to evaluate its clinical significance in preoperative staging;By comparing adjuvant chemotherapy with a single scheme and a scheme with more chemotherapy alternately, Observe their impact on their disease-free survival (DFS), provide reference for clinical choice of the mode of postoperative adjuvant chemotherapy. Methods:Our sample was consisted of47cases who had done gastric cancer resection in the first affiliated hospital of anhui medical university during October2009to November2012,They have done the EUS examination within one week before they do the operation. collecting T/N staging of EUS examination result, collecting the postoperative pathologic T/N staging at the same time, with postoperative pathologic results as the gold standard, compared with preoperative EUS examination results, in order to evaluate the accuracy of EUS examination result in T/N staging. At the same time collected Adjuvant chemotherapy clinical data of101patients who had made gastric cancer radical prostatectomy in our hospital between January2009and October2012, Analysis the affect of DFS between a single combination chemotherapy and multiple sequential joint scheme. Results:Through the EUS findings compared with postoperative pathological results, on the T staging, accuracy of EUS examination is70.2%.Tl:81.8%,T2:80%,T3:63.6%,T4:66.7%.There exist a phenomenon of early or late stages; On the judgment N staging, accuracy of EUS examination is74.5%, N-77.8%, N+72.4%, There is a shortcoming in identify the inflammatory swelling of lymph nodes and metastatic lymph nodes. The DFS in the patients who had accepted a single combination chemotherapy and multiple sequential joint scheme rspectively is:11.5months and12.7months, after SPSS19.0statistical software, found that there was no statistically significant difference oConclusions:EUS in the preoperative T and N staging of gastric cancer with high accuracy, Gastric cancer radical surgery for single scheme with multiple alternate treatment, there was no statistically significant difference in PFS in gastric cancer patients with radical prostatectomy who had done single combination chemotherapy or multiple sequential joint scheme.
