Object:Analyse and discuss with reviewing the second affiliated hospital of kunming medical university nearly five years of the primary liver disease center PBC patients with clinical manifestations and autoantibodies associated with the characteristics,and put into use the Mayo score to assess their clinical value。Methods:Collect nearly five years in the second affiliated hospital of kunming medical university liver center hospital diagnosis PBC patients clinical data,(part Ⅰ):Extract from217cases99cases (hospital for the first time, the PBC related check) the clinical performance and the experimental index as the research object, analyzing the characteristics of the major clinical manifestations; Cases and then divided into two groups of men and women, respectively, Do the statistics clinical manifestations between groups, whether there is statistical difference; Do the statistics of PBC’s major autoimmune antibody positive rate;(part Ⅱ):Compare rankly with49cases of PBC (divided into:Ⅰ,Ⅱ Ⅲ,Ⅳ) pathological classification and the Mayo score (divided into<4group,4~6group,6~8group,>8group);For the liver tissu pathology of99patients classification, with the comparison between different autoantibody positive group and negative group liver tissue pathology classification level difference, analyze autoantibodies associated with severity of liver tissue; Compare rankly with49cases of PBC (divided into:I, Ⅱ Ⅲ,Ⅳ) pathological classification and the Mayo score (divided into<4group,4~6group,6~8group,>8group); The PBC patients the main clinical manifestations such as jaundice, pruritus, fatigue, poor appetiteare divided into positive group and negativegroup,withcomparing the Mayo score whether there are statistical differences between groups, in order to make clear the main clinical manifestations of the illness and prognosis evaluation of meaning; Results:(1) clinical manifestations and main characteristics ofautoantibody:1.In the study of99patients, the male to female ratio is27:72, and average age (54.04+13.3), the clinical manifestation mainly jaundice (all are visible to the naked eye), accounts for about59.6%(59), and other such as weak31.2%(31), pruritus(22)22.2%, poor appetite(19)19.2%, signs are mainly composed of splenomegaly is about43.4%(43), ascites (30)30.0%, hepatomegaly (19) with19.2%but there are also (3)3%of patients without clinical symptoms,8%(8) cases only for abdominal distention, epigastric ache symptom such as bloody. The major clinical manifestations of comparison between men and women at the same time there was no statistically significant difference (P>0.05).2. PBC correlative autoimmune antibody:in serum of99cases were autoimmune antibody test results conclud that the AMA-M2were highest, about88.9%(87), and ANA accounted for about45.4%(45), Gp210and SP100respectively28.9%(28) and28.9%(19), for LKMS, Lc-1, SLA three kinds of antibody positive rate, only1%of Lc-1and SLA (1).(2)The application results of the Mayo score:1. PBC patients pathologic classification groups each with Mayo score between Z values (P<0.05) was statistically significant and positively correlated relationship.2. In99cases of PBC pathological classification can be divided into six groups (Ⅰ Ⅰ-Ⅱ Ⅱ,Ⅱ-Ⅲ,Ⅲ,Ⅲ-Ⅳ,Ⅳ), Gp210and SP100pathologicals rank classification between the positive and negative groups was no statistically significant difference(P>0.05).3. The Mayo score of gp210positive group and negative group’s difference was statistically significant, P<0.05, while the AMA-M2and SP100has no statistical significance between the positive and negative groups,(P>0.05)。4.99cases of PBC patients, pruritus and jaundice group with no pruritus and jaundice’s Mayo score (P<0.05) difference was statistically significant. About to weak and poor appetite’s Mayo scores between the groups with and without weak and poor appetite, there was no statistically significant difference (P>0.05)。Conclusion:1) PBC clinical manifestation and main autoantibody features:1.99patients with PBC were:the most prominent is the signs and symptoms of jaundice and splenomegaly, followed by weak and pruritus, poor appetite is the most rare. There is no difference between men and women the main clinical manifestations.2. Resistance to AMA-M2is the most specific antibody.2) the Mayo score application results:And can reflect the liver tissue of the PBC disease pathological staging, GP210PBC related antibodies and clinical presentation (jaundice, itching), compared the Mayo score more can reflect the PBC comprehensive, accurate and sensitive disease.
