ObjectivesTo evaluate the incidence of abnormal behaviors in pervasive developmental disorders(PDDs) and to assess the efficacy and adverse reactions of risperidone titration treatment, which can provide the physicians with data of abnormal behaviors of PDDs and guild individual treatment programs in clinical practice for PDDs patients.Methods93of2~15years old ICD-10criteria defined PDDs patients were assessed their abnormal behaviors by using Aberrant Behavior Checklist and Conners Index of Hyperactivity (CIH;. Their parents’emotional state, parenting stress and quality of life were evaluated by the Hospita:Anxiety and Depression Scale(HAD).Caregiver Strain Questionnaire(CGSQ) and The World Health Organization Quality of Life-100(WHOQOL-100). At the same time the informations of the patient’s clinical features including gender, age. parents’ emotional stau and stress level first onset of symptoms generally etc. were also collected. Patients with serious behavior problems underwent7weeks systematic risperidone titration treatment according to their weights, with Aberrant Behavior Checklist and CIH assessing drug efficacy, Treatment Emergent Symptom Scale (TESS) assessing adverse reactions.Results1. Children with asperger’s syndrome(AS) were more likely than children with autism to get education opportunities(P=0.000); For the initial symptoms of disease, Speech stunting was more common in autism while social communication disorders and stereotyped sounds/speech were more common in AS children(P<0.05).2. The scores of Irritability and Self-injury in male patients were higher than those of female patients(P=0.036; P=0.000), and autism patients had more seriously self-injury behaviors than AS patients (P=0.009).3. In the study, the incidence of aberrant behaviors was61.30%for hyperactivity,12.90%for self-injury,36.60%for stereotyped action,4.30%for stereotyped sounds/speech,59.10%for both stereotyped action and sounds/speech, and54.8%for irritability in PDDs patients.4. PDDs patients with hyperactivity behavior would increase caregivers parenting stress (P=0.035). Parents of patients with stereotyped action would have the most serious depression mood (P=0.028) and the worst quality of life (P=0.041) compare to patients with stereotyped sounds/speech and both stereotyped action and sounds/speech.6. Risperidone titration treatment was effective and well tolerated for the treatment of hyperactivity and irritability in children with PDDs. especially effective for hyperactivity. Appetite increase and sedation were the main side reactions of the treatment.Conclusions1. The abnormal behaviors of PDDs patients were common in clinical practice, and hyperactivity and irritability were the most common ones. Male were more likely to having irritability and self-injun behaviors and Autism had more seriously self-injury behavior than AS.2.PDDs patients with abnormal behaviors would increase parenting stress, negatively influence emotion and reduce quality of life of caregivers.3. Risperidone titration treatment was effective and well tolerated for the treatment of Hyperactivity and Irritability in children with PDDs. Appetite increase and sedation were the the main side reactions of the treatment. However,, the conclusions still need to be confirmed in the randomized double-blind controlled drug test of large sample size due to the small sample size in the study.
