Correction Analysis Of Cancer-related Fatigue And Quality Of Life Among The Patients With Colorectal Cancer After Surgery

ObjectiveTo describe the status about CRF in colorectal cancer patients after surgery; to explore the relationship between the quality of life and cancer-related fatigue; providing theoretical basis for intervention of CRF in postoperative colorectal cancer patients and reference for better assessment and treatment.MethodsA descriptive and relative research, carried out in a hospital in Tianjin by using questionnaires of Cancer Fatigue Scale (CFS) and the quality of life Scale (EORTC QLQ C30). The patients whose clinical diagnosis was colon or rectal cancer, who were1year after operation living in hospital during June2012to March2012, were investigated. Basic information, clinical conditions and the scales of quality of life and CRF were described by frequency, percentage, mean and standard deviation. The relationship between CRF and QOL was tested by correlation analysis. SPSS17.0software package was used for statistical analysis.Results(1) The overall score of QOL of patients with colorectal cancer after the surgery was51.51±19.13, lower than reference values which recommend by EORTC. Among the functional scale, all of the scores were lower than the reference, and the score of cognitive function was the highest, while the role function was the lowest. Among the syndrome scale, all of the scores were higher than the reference, and the score of fatigue was the highest. Finally, in those single items, the scores of item insomnia, diarrhea and economic difficulties were higher than the reference, and the score of economic difficulties was the highest.(2) The incidence of CRF in these patients was91.82%, the overall scores was34.77±9.48, the scores of dimensions from hingh to low in turn as follows:physical fatigue, affective fatigue and cognitive fatigue respectively. The highest score item among physical fatigue was "want to lie down and rest", among cognitive fatigue was the item "feeling thinking become slowly", and among affective fatigue the lowest one was "feeling full of energe". (3) The CRF in colorectal cancer patients after surgery correlated negatively with the whole QOL (r=-0.480~0.570, p<0.05),and also correlated negatively with the functional scales of QOL (r=-0.095~-0.428, p<0.05), while correlated positively with the symptom scales(fatigue, nausea, vomiting, pain, of QOL ((r=0.262~0.638, p<0.05), and also correlated positively with the insomnia r=0.322-0.363, p<0.05), economic difficulties (r=0.304~0.361, p<0.05)Conclusion(1) The overall quality of life and functional levels of patients with colorectal cancer after the surgery were down,belongs to medium level. Among the syndrome scales, the level of fatigue was the highest. In those single items, the economic difficulty in those patients was obvious.(2) The incidence rate of CRF in colorectal cancer patients after surgery was higher, and most suffered from moderate level. Physical fatigue was more serious than affective fatigue and cognitive fatigue.(3) Cancer-related fatigue correlated with the overall quality of life of patients with colorectal cancer after the surgery. The patients who experienced severe cancer-related fatigue would have poor quality of life, and their functional status also became lower. If the patients experienced symptoms such as pain, insomnia, nausea and vomiting, they would feel more fatigue.
