Objective To analysis the expression of immune inflammatory factors like interleukin(IL)-17and IL-8in chronic gastritis, lymphocytic gastritis, gastric mucosa-associated lymphoid tissue (MALT) lymphoma and gastric MALT lymphoma after eradication of Helicobacter pylori (H. pylori) and investigate the correlation between them. Meanwhile, to analysis the effect of H. pylori on the expression level of IL-17and IL-8, in order to explore the function and significance of IL-17and IL-8in gastric MALT lymphoma.Methods59patients who all received gastroscopy and biopsy were enrolled in the study from endoscopy room of the General Hospital, Tianjin Medical University from May2008to May2012. They were divided into groups of chronic gastritis, lymphocytic gastritis and gastric MALT lymphoma according to pathology. H. pylori was detected by13C-Urea Breath Test (UBT) in all patients. According to these results, the patients were divided into H. pylori positive and negative group. In addition,14gastric MALT lymphoma patients who all received eradication of H. pylori were also enrolled in the study. To detected the expression of IL-17and IL-8by immunohistochemical staining and then compare the different level of among the4groups. Meanwhile, to analysis the relation between H. pylori and the expression level of IL-17and IL-8and further explore the correlation of IL-17and IL-8in originate of gastric MALT lymphoma. All statistical analysis were carried out with SPSS17.0and P<0.05was considered significant.Results (1) The composition of sex, age and H. pylori was no significant difference in different groups.(2) The expression of IL-17increased in chronic gastritis, lymphocytic gastritis and gastric MALT lymphoma cases, corresponding to (11.9±2.4),(14.7±3.6) and (16.2±3.3) per high magnification field, respectively. The levels of IL-17in gastric MALT lymphoma and lymphocytic gastritis were significantly higher than that in chronic gastritis (P<0.01). However, no statistical significance was found in IL-17expression between lymphocytic gastritis and gastric MALT lymphoma (P=0.136).(3) The increased expression of IL-8in lymphocytic gastritis and gastric MALT lymphoma cases (corresponding to21.0±4.7and21.8±4.6 per high magnification field, respectively) had no statistical significance(P>0.05), but they were all higher than that in chronic gastritis (14.4±4.6per high magnification field)(P<0.01).(4) The levels of IL-17and IL-8expression were obviously down-regulated in gastric MALT lymphoma after eradication of H. pylori (corresponding to12.8±2.5and18.9±2.7per high magnification field, respectively), comparing with that in gastric MALT lymphoma (P=0.001,P=0.011, respectively).(5) The expression level of IL-17and IL-8were significantly higher in H. pylori positive group than in negative group (P=0.012,P=0.006).(6) The expression of IL-17was positively correlated with that of IL-8in all groups, and in gastric MALT lymphoma, r=0.681,P<0.01.Conclusion (1) IL-17and IL-8up-regulated in the development of gastric MALT lymphoma suggested that IL-17and IL-8played an important role in the development of gastric MALT lymphoma possibly.(2) After eradication of H. pylori, the levels of IL-17and IL-8expression were obviously down-regulated in gastric MALT lymphoma. It is possible that IL-17and IL-8play an important role in originate of gastric MALT lymphoma together with H. pylori.(3) The expression of IL-17and IL-8between gastric MALT lymphoma and lymphocytic gastritis was no statistical significance. Maybe Lymphocytic gastritis was closed related with gastric MALT lymphoma, it may be the precancerosis of gastric MALT lymphoma.(4) The expression of IL-17and IL-8was positively correlated, which suggested they could promote the expression each other through direct or indirect way and had a synergistic effect.(5) IL-17and IL-8may become the gene targets for treatment and early intervention of gastric cancer and precancerous lesions through genetic engineering technology.
