BackgroundPanic Disorder(Panic Disorder, PD), is a recurring significant plant nerve symptoms such as palpitations, sweating, shaking, with strong sense of dying or out of control, fear of unfortunate consequences of hair as a characteristic of an acute anxiety Disorder,and is one of the most common psychological diseases.International collaborative research report of15centers of WHO showed1.1%in comprehensive hospital in patients with PD,and the disease accounted for6%27%in psychiatric patients.Mental disorder epidemiological survey in Hebei province in2007found that PD prevalence was3.96%o.The results was higher than1.76%o reported in Zhejiang province in2005.Study found that patients with PD merger suicide or suicide attempts ratio of2times that of other mental disorders and20times without mental illness.Merger of substance abuse, alcohol abuse is common in patients with PD, personality disorder and so on.WHO reported PD as one of the world within the scope of major psychiatric disorders, the percentage of disease burden in column5, seriously affecting the patient’s mental health, quality of life, and life safety in2001.On this basis. the study of PD is very important and urgent.Modern clinical commonly use therapy.Including cognitive behavioral therapy and psychodynamic therapy and morita therapy. EMDR and other treatment.Drug intervention is another way. But at present, the international reports that antidepressants,such as benzodiazepines, beta adrenergic receptor blockers, selective serotonin reuptake inhibitors, etc., are dependent, slow onset of action, interactions between drugs and adverse reactions, etc.So many scholars devoted to seeking new methods, new means of PD treatment.Traditional Chinese medicine gradually came to the attention of some scholars.There is no the word "panic disorder" in Traditional Chinese medicine.From the clinical perspective,the disease is an emotional disease,which belongs to heart disease.But, at present the standardization of Chinese medicine treatment of the disease were not seen authority reported both at home and abroad, this topic group in recent years has been committed to studying on the TCM etiology and pathogenesis of the disease and its treatment of traditional Chinese medicine,and followwing up the patients.Patients can quickly relieve clinical symptoms, and treatment result is quite satisfactory. So, we think it is necessary to gradually improve the treatment of traditional Chinese medicine, Chinese medicine diagnosis and treatment of panic disorder scheme, diagnostic criteria of traditional Chinese medicine and curative effect evaluation standard,and form a specification of individualized treatment plan and promotion.For further clinical application, the patients will be free from suffering and economic burden.ObjectivesThis study to systematically sort of panic disorder at home and abroad research results, summarized this topic group’s clinical experience in the treatment of panic disorder in recent years, research results and effective agent.This topic group study panic disorder therapy of traditional Chinese medicine, Chinese medicine diagnosis and treatment methods, TCM diagnosis and curative effect evaluation standard, in order to further optimize the treatment of panic disorder in clinical curative effect.MethodsThis research adopts the central multicenter randomized controlled research methods.According to the inclusion criteria, this research collected from June2009to August2009in Wangjing hospital of traditional Chinese medicine academy of sciences, Beijing university of Chinese medicine in China Eastern Chaoyang hospital, capital medical university hospital in patients with panic disorder.Traditional Chinese medicine treatment group is used the method of benefit your heart and gallbladder, calming nerves.Daily1agent, points taken twice in the morning and evening, using free decoction granules;The western medicine group is treated with paroxetine hydrochloride.Daily1agent, points taken once in the morning.One course of treatment is seven days in both groups, and every group continues four courses of treatment, which are a total of four weeks. Both groups are followed up for3months, totaling4months.this research Uses Hamilton anxiety scale (HAMA) to evaluate the clinical effect and side effect scale (TESS) to assess adverse reactions.The study income group163patients, including13cases fall off, out of7cases, to complete cases,143cases(The group of76cases of Chinese medicine, western medicine group76examples).Results1.Two groups of baseline comparisonPatients’demographic characteristics such as age, gender, height, weight, vital signs, clinical course, past medical history and history of allergies, Hamilton anxiety scale (HAMA) in the distribution of differences between groups have no statistical significance (P>0.05). Two groups are comparable.2.1n the two groups before and after treatment and follow-up curative effect comparison2.1In the two groups before and after treatment the Hamilton anxiety scale (HAMA) scoreHAMA total score curative effect of Two groups before and after treatment were statistically analyzed.Since treatment of day1, HAMA scores of the experimental group patients before and after improvement the difference was statistically significant (P<0.05);From treatment for3days, the control group in patients with HAMA overall score before and after improving the difference was statistically significant (P<0.05).HAMA scores improved from the treatment of patients with3days of the test group were better than control group (P<0.05), meanwhile the tension of the experimental group, the symptom scores improve sensory system is better than that of control group (P<0.0);Since treatment for five days, group cardiovascular system symptom of patients, plant nerve symptom, interview, behavior score improvement is better than that of control group (P<0.05).2.2Two groups’ follow-up HAMA scale overall curative effect comparisonAt follow-up4weeks in the control group HAMA scale overall efficiency is higher than experimental group curative effect, still can’t believe that there are differences between the two groups (P>0.05). Efficient in8and12weeks follow-up test group is higher than the control group, still can’t believe that there are differences between the two groups (P>0.05).2.3In the two groups before and after treatment clinical curative effect comparison On the basis of therapeutic effect evaluation standard of HAMA,4weeks after treatment, the treatment group has82cases, including43cases of clinical cure,24cases being markedly effective, effective9cases, ineffective6cases, and the total effective rate is92.68%; Control group has74cases, including18cases of clinical recovery,30cases being markedly effective, effective in16cases, ineffective10cases, and the total effective rate is86.49%.3The safety of the two groups before and after treatment comparison3.1In the two groups before and after treatment of adverse events1case of adverse events in the control group and6cases of adverse events in the treatment group that donot thinked the proportion of adverse events is similar between the two groups (P>0.05).Both are not corresponding treatment measures, and Two groups of patients symptoms are alleviated.3.2Comparing the status of the two groups before and after treatment side effectswith14days after treatment,Experimental group occurred in1case of dry mouth and1case of nausea and vomiting; control group occurred in14cases of dry mouth,and10cases of nausea and vomiting.Occurrence of adverse reaction rate of the control group is higher than experimental group (P<0.05).with28days after treatment,Experimental group occurred in2case of constipation,without dry mouth; control group occurred in11cases of dry mouth.and11cases of constipation. Occurrence of adverse reaction rate of the control group is higher than experimental group (P<0.05). Conclusions1. Traditional Chinese medicine (TCM) prescription, from the heart liver differentiation treating panic disorder has good curative effect, high recovery rate, and is better to improve the symptoms of stress, sensory system, cardiovascular system symptoms, plant nerve symptoms.2. Traditional Chinese medicine treatment of panic disorder in the working time is fast, and the recurrence rate is low.3. adverse reactions of Chinese medicine treatment of panic disorder is small, such as incidence rate of dry mouth, constipation, nausea and vomiting incidence is low.4. This study shows that the occurrence of panic disorder is closely related to the heart, courage.Heart and gallbladder qi deficiency in the occurrence of panic disorder plays an especially important role.
