Objective:This thesis compares data differences of temperature measurements of neonates inNICU incubators at different parts of body which are armpit and back interscapularusing mercury thermometers which are of same standard at same time period. Usingback interscapular as the part of body for temperature measurement can improve themeasurement accuracy and can avoid the security risks of the mercury thermometerbroke by accident and is good to the recovery of neonates patients and to theimprovement of the overall quality of care. Therefore, if the measurement data has nostatistical difference we can use back interscapular as the best part for temperaturemeasurements for neonates in NICU incubators and promote it in application.Methods:1. objectsThe subjects of study were selected from the NICU of the third affiliated Hospitalof Zhengzhou University between October2011and December2011. All baby patientsselected stayed in NICU incubators because of low weight that are below2kg. And allbaby patients selected aged between0.17hour and480hours which averaged26.63hours and weighted between0.8kg and1.98kgs which averaged1.76±0.38kg.2. toolsTemperature measurement tools for all neonates in NICU incubators are mercurythermometers provided by Shanghaihuachen Biomedical Instrumentation Co,LTD. Thetemperature error is less than0.1℃which is acceptable. Routine disinfection areemployed before and after measurement.3. measuring method(1) the thermometer verification: Using KR-3C thermometer shaking apparatusproducted by Jintan XinHang instrument factory to shake all mercury thermometers used in temperature measurement to make sure temperature of mercury thermometers isbelow35℃, and at the same time put thermometers into water of40℃. After fiveminutes get all thermometers out at the same time and check the temperature. If any ofthem has error of0.1℃above, or has split in the glass tube or mercury column of anythermometer declines automatically then it will not be used for measurement.(2) measurement conditions: Avoiding measurement when neonates in NICUincubators are crying or eating or being provided other health care operations, we canmeasure after30minutes if these happen.(3) operation method: After washing hands, open the door of the warm box gently,take neonatal in supine position and remove the pillow. Pick up two thermometers. Oneinserted into the spine and put it between trapezius muscles parts about4.5-6.5cm. Twomeasurement parts of the measuring time are10minutes, after10minutes in orderaccording to two of the thermometer out respectively, to the eyes keep reading andrecord the horizontal.(4) disinfection methods: Use the thermometer into disinfection liquid containers inthe water, take out after5minutes into the double and lining the container with thescreen hole, rinse, use electric jilt table machine will be the mercury thermometer jilt to35℃below, then into another disinfection container for30minutes, remove and rinseagain and put in dry clean containers.4. statistical methodAll datum using numerical SPSS17.0statistical data to analysis. All measured saidby numerical mean differences±standard deviation (X S),two areas betweeninterscapular area and under their arm of the data measuring temperature t test andPearson ’s correlation analysis. Two different sexes monitoring site between babycompares the two independent t test sample; All the children age and monitoring sitebetween the Pearson correlation analysis.Results:1. Participate in the study were201children, choose under your armpit asmeasuring temperature parts, each time needed for children for30minutes (0.5hours),and all of them there need to6030minutes (100.5hours); Choose back interscapulararea as measuring temperature parts, each time needed for children for30-90seconds,and all of them to6030-18090seconds altogether (1.67-5.02hours).2. All the back interscapular area of measuring temperature is37.0592±0.4533℃,the highest temperature is38.60℃, the minimum temperature is35.80℃. All the armpit measuring temperature is37.0617±0.4499℃, the highest temperature is38.60℃, theminimum temperature is35.70℃.3. All arms and back interscapular area of measuring temperature value ofstatistical analysis of the match. The results showed all children’s armpit and backinterscapular area between measuring values were no diversity.4.201cases of children under their armpit and back interscapular area ofmeasuring temperature value between gender statistical analysis, the results showed thattwo monitoring site between the numerical male, the female children were no statisticaldifference.5. All of them back interscapular area and two parts under the measured valueadopt correlation analysis, the results show that all the children of the two parts of themeasured values are in highly relevant.6. All the children age and their two parts measured value of under armpit and backinterscapular area adopt related analysis, the results showed that the measured value ofbetween the patient’s age and two parts is not correlation.Conclusions:1. The temperature measurement of the neonates in NICU newborns choose backinterscapular area and two parts under the measure numerical no difference, so the backinterscapular area can instead of the armpit.2. The neonates in NICU incubator choose back interscapular area as monitoringsite, compared to the conventional under your armpit and the whole measuring processshorten the continuous warm box of the open time, be helpful for children of theconstant temperature box warm box, illness recovery and to the protection of childrenisolation.3. The neonates in NICU incubator choose back interscapular area measurementtemperature, the operation method is simpler, greatly reduce the nurse implementationof the measure temperature and reduce the time work load, be helpful for the quality ofholistic nursing care in NICU.
