Objective: To investigate the clinical features of drug-induced liver disease(DILD),standardize the diagnosis of drug-induced liver disease, evaluate the Danan and Mariadiagnostic criteria for clinical applications, in order to improve people the understandingand the level of diagnosis and treatment of drug-induced liver disease.Methods: drug-induced liver disease patients admitted to the First AffiliatedHospital of Dalian Medical University from2007-2011were reviewed retrospectively.the etiology, clinical manifestations and treatment outcomes were recorded. Spearmanrelative analysis and paired test were used to evaluate the consistency and clinicaljudgment accuracy of Danan and Maria diagnostic criteria.Results:1.The general situation: The are46male patients and61female patientsin107cases.11-year-old minimum age, maximum age84years old, the average age is46.62years old with a disability16.76.2.Pathogenic drugs: The most common drugs causing liver damage are those asfollow: herbal drug(39.3%), antineoplastic agents(11.2%), antithyroid drugs(8.4%),antibiotics(6.5%), and anti-tuberculous drugs(4.7%).3.Clinical manifestations: mainly for the65cases of weak (60.7%), yellow color ofurine52cases (48.6%),51cases of jaundice (47.7%), nausea in50cases (46.7%), fever40cases (37.4%),33cases of abdominal distention (30.8%), vomiting in22cases(20.6%),18cases of skin itching (16.8%),14cases of skin rash (13.1%).4.Clinical classification: there are81cases of liver cell type(75.7%),12cases ofcholestasis type (11.2%),14cases of mixed type (13.1%)in107patients.5.Comparing with the two clinical diagnostic scales by Spearman correlationanalysis, it showed that they had positive correlation. The Danan’s scale showed better discriminative power and produced assessments closer to clinical determinant.6.In the total107cases, the cure, improvement and corruption ration were17.8%,40.2%,and42.1%,respectively.The cure and improvement rate of liver cell type,cholestasis type and mixed type was53.1%,75.0%,71.4%, respectively, p=0.196,P>0.05.Conclusion:1.The are various of medicine dued to DILD, while herbal drugs,antineoplastic agents, anti-thyroid drugs was predominated.2.The liver cell type was common, the mixed type was less and while thecholestasis type was the least one.3.Comparing with the two clinical diagnostic scales by Spearman correlationanalysis, it showed that they had positive correlation. The Danan’s scale showed betterdiscriminative power and produced assessments closer to clinical determinant.
