Objective: To investigate the feasibility of miniplate fixed outside of mucosa andclinical application.Methods: This study selects the three adult mongrel dogs as experimental animals,fixes the miniplate to the mucous membrane as the anchorage by planting nails inTaiwan and South Korea split planting nails (JNH) nails in the bilateral maxillary molararea, and tracts the third incisor to the distal with chain-shaped aprons (150g/350g),replacing the chain ring every four weeks for three times. Through the generalobservation, model measurement, X-ray and CT imaging and soft tissue HE staining,this study aims to analyze and evaluate the feasibility of retention and clinicalapplication of the miniplate as the anchorage outside of mucosa, and to explore themethods of reducing mucosal inflammation and modifying titanium plate structure.Results:1. general situation and visual observationFixing the titanium plate to the maxillary of the dogs by use of planting nails, dogscan eat normally with no adverse reactions and normal weight gain. There are no suchsituations as implants and titanium plate coming off, shifting and so on, and the cleargap between the second incisor and the third incisor for12weeks. However, all aroundthe planting nail appear varying degrees of inflammation, among which the loosetitanium plate in group B oppresses the mucosa, resulting in the more obviousinflammation in the local gums and buccal mucosa. Around the titanium plate with noloosening, there is a marked chronic inflammatory reaction and soft tissue partiallysurrounding the miniplate anchorage.2. storage model measurementMeasurement of the three dogs storage models before and after the experiment with the result that I3cusp spacing is in an average increase of1.27mm, I3and Cspacing in the cervical averagely decreasing1.05mm and in the tooth tip averagelydecreasing1.76mm indicates that the third incisor of the left and right side obviouslyhas the distal and labial tipping movement.3. X-ray, CT image analysisX-ray shows that there are no shadows around the planting nails which have goodcombination with the alveolar bone. Titanium plates are stable without shift, andplanting nails and titanium plate coverage areas show no significant cortical boneabsorption, which indicates that mucosal inflammation has no impact on the stability ofthe planting nails and the adjacent bone tissues. However, I3is subject to greatertraction to move blocked by the canine root and result in moderate root absorption.4. histological stainingJNH nail fixed titanium plate is stable and no loose, inflammation mainly soft tissuefibrosis, showing that large amounts into the the fibroblasts fiber cord and the moderatenumber of inflammatory cells; loose titanium plate oppression of the gingival mucosa, andplanted the nail head protruding part of the angularmore obvious friction buccal ulcers,inflammatory reaction, showing that a large number of inflammatory cells invasion.Conclusion:1. Animal experiments show the feasibility of miniplate fixed outside of mucosaas the anchorage and clinical application.2. Two planting nails’ retention of miniplate can not only significantly improvethe initial stability of the anchorage and anchorage effect, but also avoid the case ofdamaging tissues owing to too much focus of a nail’s retention stress.3. The miniplate fixed outside of mucosa can simplify SAS implantation, reducingthe adverse reactions such as swelling, pain and others during the implanted earlysurgical flap surgery.4. Findings of the experiment: Due to the retention between the two nails easier tosave the food, the higher retention position of the miniplate, the mucosa’s easiness tocover and hardness to clean, there appear chronic inflammatory reactions in the buccalmucosa. Therefore, in clinical applications, the structures of the titanium plates and thesolid nails need to be adjusted in order to reduce the food residues and mucousmembrane irritations.
