Analysis On Clinicopathologic Features In Different Age Group Patients With Breast Cancer

Objective:To analyze the differences of the clinicopathologic characteristics indifferent age groups of patients with breast cancer in order to provide more informationfor individualized treatment.Methods:We retrospectively analyzed the clinical and pathologic features of410breast cancer patients who had received operation treatment and were confirmedpathologically after surgery from January2003to December2004in our hospital.According to their age, the patients were divided into four groups:≤35yearsgroup(young-age group),36-50years group,51-64years group and≥65years group(old-age group).The data of the four groups were compared with Chi-square test.Results:There were statistical differences in the TNM staging, lymph nodemetastasis rate, operation mode,complications(Diabetes, Hypertension, Heart disease)and the5-year survival status between the four groups(P<0.05). Proportion ofstageⅠin≤35years group was21.2%(7/33),in36-50years group was14.4%(29/201),in51-64years group was5.6%(8/142)and in≥65years group was11.8%(4/34),respectively(χ~2=9.39,P<0.01). Proportion of stage Ⅱand Ⅲ in the fourgroups had no significant differences(P>0.05). The lymph node metastasis status in≤35years group were showed as followed:the lymph node positive rate was48.5%(16/33),the patients with1-3lymph nodes metastasis accounted for30.3%(10/33),≥4lymph nodes metastasis accounted for18.2%(6/33).The lymph node metastasisstatus in36-50years group were showed as followed:the lymph node positive rate was57.2%(115/201), the patients with1-3lymph nodes metastasis accounted for37.3%(75/201),≥4lymph nodes metastasis accounted for19.9%(40/201). The lymph nodemetastasis status in51-64years group were showed as followed:the lymph nodepositive rate was58.5%(83/142), the patients with1-3lymph nodes metastasis accounted for28.9%(41/142),≥4lymph nodes metastasis accounted for29.6%(42/142). The lymph node metastasis status in≥65years group were showed asfollowed:the lymph node positive rate was76.5%(26/34), the patients with1-3lymphnodes metastasis accounted for52.9%(18/34),≥4lymph nodes metastasis accountedfor23.5%(8/34).Statistics analysis showed that:the four groups had no significantdifferences in lymph node positive rate,but compare the data in≥65years group withthat in≤35years group,36-50years group and51-64years group,it showed significantdifferences(χ~2=6.55,P<0.05;χ~2=5.37,P<0.05;χ~2=4.36,P<0.05). The fourgroups had significant differences in1-3lymph nodes positive rate(χ~2=11.98,P<0.01),but there were no significant differences in≥4lymph node positive rate(P>0.05).Proportion of non-radical operation was27.3%(9/33),18.4%(37/201),9.2%(13/142) and23.5%(8/34)in≤35years group,36-50years group,51-64yearsgroup and≥65years group respectively(χ~2=10.16,P<0.05).Proportion of the patientswith complications was0%(0/33),4.5%(9/201),13.4%(19/142)and50%(17/34)in≤35years group,36-50years group,51-64years group and≥65years grouprespectively(χ~2=66.59,P<0.01). The5-year survival rates had no obvious differencesin the four groups﹝≤35years group:78.8%(26/33),36-50years group:78.6%(158/201),51-64years group:76.8%(109/142),≥65years group:73.5%(25/34).(χ~2=0.07,P>0.05)﹞.The5-year mortality rates in the four groups showed significantdifferences﹝≤35years group:3.0%(1/33),36-50years group:1.5%(3/201),51-64years group:9.2%(13/142),≥65years group:8.9%(3/34),(P<0.01)﹞. Among thefour groups,there were no significant differences in the tumour size and the positiveexpressions of ER and/or PR,ER,PR and Her-2(P>0.05),but there were statisticaldifferences in the ER positive rate between the≤35years group and the≥65years group﹝≤35years group,45.5%(15/33)VS≥65years group,76.5%(26/34)﹞,the PRpositive rate between the36-50years group and the51-64years group﹝36-50yearsgroup,63.2%(127/201)VS51-64years group,50.7%(72/201)﹞,the Her-2positiverate between the≤35years group and the51-64years group﹝≤35years group.9.1%(3/33)VS51-64years group,25.4%(36/142)﹞.Conclusion:In the young-age group,most of the patients have been discovered atthe early stage,the proportion of stageⅠwas the highest, the lymph node positive ratewas the lowest;In the old-age group,patients had been staged later when they werediagnosed, the lymph node positive rate was the highest, and most of the number ofmetastatic lymph nodes was1-3; The ER positive rate in the old-age group was much higher than the young-age group, the PR positive rate in the36-50years group wasmuch higher than the51-64years group, and the Her-2strong positive rate in the51-64years group was much higher than the young-age group;The young-age group’sproportion of non-radical operation was the highest, the old-age group ranked secondly,the51-64years group’s proportion of non-radical operation was the highest; The5-yearmortality rates of the patients above50were higher than the other two groups; Theolder the age,the higher the proportion of the patients with complications.
