Objective: Cerebral infarction is due to cerebral atherosclerosis, stenosis orhemodynamic changes, as well as coagulation dysfunction induced by brain bloodsupply disorder, ischemia, hypoxia caused by focal cerebral softening, necrosis.Cerebral infarction incidence is about110/100000of the population, accounting forabout60%~80%of all stroke, which is about50%of the survivors have differentdegrees of dysfunction,such as limb movement disorder, sensory disorders, languagebarriers, and even long-term coma showed vegetative survival.Even after regular drugtreatment and rehabilitation treatment, also did not make the most of patients distressfrom cerebral infarction sequelae. Looking for a treatment method which is in order toimprove the treatment effect of sequelae of cerebral infarction, the greatest degree ofreduced cerebral infarction sequelae in patients with disability, Improving the lifequality of the patients,Which have become an important topic in the research of neuralscience. In recently years, the rapid development of stem cell therapy have provided abrand new way for cerebral infarction treatment. The study is designed to investigate theclinical application problems of cord blood stem cells transplantation in the treatment ofcerbral infarction sequela period by observing its treatment effect.Methods:1.subjects: Dalian Municipal Central Hospital had treated60inpatients withcerebral infarction sequela patients from the March of2010to the November of2011,including42cases of male patients,18cases of female patients, the age is34to84years old, and the clinical course of0.5~14years.Prior to the treatment in the60inpatients,the following lab tests were performed:blood routine,blood type,urine routine,hepatic function,renal function,bloodglucose,blood electrolytes (sodium,potassium, chloride), Hepatitis virus and tumor markers,ECG, chest radiography, CT orMRI.The test excluded the patients with malignant tumor, severe heart and lungfunction and liver and kidney dysfunction, and the infection patients who can notcomplete the research content or cannot cooperate with investigators.The60patients were randomly divided into control group and umbilical cordblood stem cells treatment group,30cases/group.It were not significant differences inthe two groups of patients with age and disease duration (P>0.05), that is comparable.2.methods:The control group:the patients had routine rehabilitation trained underthe guidance of the rehabilitation therapist in the hospital, including the normal limb putin the positiont,the limb joints move passivly and activly, turning over and sitting up inthe bed, standing and walking training, activities of daily living training.The umbilical cord blood stem cell treatment groups:the group treat on the basis ofthe control group,and the patients and their families agree on and sign the consent,giving the umbilical cord blood stem cells treatment of1time/week, a total of2times,Interval time is7~10days between the two. Infusing with stem cell nucleus cellsuspension through a peripheral vein,the total number nucleated cells had tested greaterthan1×10~8. The umbilical cord blood had come from the healthy newborn abandonedthe umbilical cord and placenta, separating the nucleated cells according to Ningxiareached in the joint biological Limited cell processing kit manual method.each patient had Scored with Fugl-Meyer motor function and evaluate with thelatest revision of Ashworth scale before the treatment and2weeks after the end oftreatment.Statistical analysis:undertake statistical processing with the software of SPSS11.5,measurement data is showed by X±S, before and after treatment and comparedbetween groups have done T test, P<0.05for the difference, it was statisticallysignificant.Results:The most patients showed obvious decrease in muscle tension,limbmotor function improved in different degrees after the umbilical cord blood stem cellsfor the treatment of the end of the2weeks, Treatment effect is good in the umbilicalcord blood stem cells treatment group than the control group. We had not find anyadverse reaction during the treatment of umbilical cord blood stem cell treatment group.Conclusion:1. umbilical cord blood stem cell have a valid effect in the treatment of the sequelae of cerbral infarction in the near future. Patients with dystonia and motorfunction were improved in different degrees,The curative effec of umbilical cord bloodstem cell therapy combined with rehabilitation training is better than the rehabilitationtraining.2. umbilical cord blood stem cell is safe in the treatment of cerebral infarction byveins. No patients were found significant discomfort and complications.
